摘  要:目的分析在慢性阻塞性肺疾病(COPD)急性加重期患者治疗过程中应用中性粒细胞/淋巴细胞比值(NLR)、血小板/淋巴细胞比值(PLR)和C反应蛋白(CRP)指标预测其是否需要机械通气的预测价值。方法从2021年1月至2023年12月在我院接受治疗的COPD急性加重期患者104例的临床资料进行回顾性分析,按照是否接受机械通气治疗分为机械通气治疗组(n=52)和非机械通气组(n=52)。比较2组患者的一般资料以及NLR、PLR、CRP指标水平,根据上述比较情况将P<0.05的变量纳入logistic回归模型中进行分析,并对NLR、PLR、CRP对COPD急性加重期患者机械通气治疗的预测价值进行分析。结果机械通气治疗组NLR、PLR、CRP指标水平均高于非机械通气治疗组(t=9.530、9.147、8.813,P<0.05)。将P<0.05的变量纳入logistic回归模型中进行分析显示,NLR、PLR和CRP是COPD急性加重期患者选择机械通气治疗的影响因素[OR(95%CI)=2.111(1.234,3.611),2.462(1.328,4.565),2.333(1.534,3.548),P<0.05]。经受试者工作特征曲线(ROC)分析显示,NLR、PLR和CRP联合预测COPD急性加重期患者机械通气治疗的价值最高(P<0.05)。其中,三者联合AUC为0.977,准确率、灵敏度、特异度、阳性预测值、阴性预测值分别为95.19%、100%、90.38%、91.23%、100%。结论NLR、PLR、CRP是COPD急性加重期患者机械通气治疗的危险因素,且上述3项指标联合对急性加重期COPD患者是否机械通气治疗具有较高的预测效能,可为患者是否选择机械通气治疗提供有效参考数据。Objective To analyze the predictive value of neutrophil/lymphocyte ratio(NLR),platelet/lymphocyte ratio(PLR),and C-reactive protein(CRP)in predicting the need for mechanical ventilation during the treatment of patients with acute exacerbation of chronic obstructive pulmonary disease(COPD).Methods Clinical data of 104 patients with acute exacerbation of COPD treated in our hospital from January 2021 to December 2023 were selected for retrospective analysis,and were divided into control group and observation group according to whether they received mechanical ventilation therapy,with 52 cases in each group.The general data and the levels of NLR,PLR and CRP were compared between the two groups.According to the above comparison,variables with P<0.05 were included in the logistic regression model for analysis,and the predictive value of NLR,PLR and CRP for mechanical ventilation therapy in patients with acute exacerbation of COPD was analyzed.Results The levels of NLR,PLR and CRP in the mechanical ventilation treatment group were higher than those in the non-mechanical ventilation treatment group(t=9.530,9.147,8.813,P<0.05).NLR,PLR and CRP were the influential factors for choosing mechanical ventilation therapy in COPD patients with acute exacerbation[OR(95%CI)=2.111(1.234,3.611),2.462(1.328,4.565),2.333(1.534,3.548),P<0.05].The analysis of receiver operating characteristic curve(ROC)showed that NLR and PLR combined with CRP had the highest value in predicting mechanical ventilation therapy in COPD patients with acute exacerbation(P<0.05).The AUC of the combined three indicators was 0.977,and the accuracy,sensitivity,specificity,positive predictive value and negative predictive value were 95.19%,100%,90.38%,91.23%and 100%,respectively.Conclusion NLR,PLR and CRP are risk factors for mecha-nical ventilation therapy in patients with acute exacerbation of COPD,and the combination of the above three indicators has high predictive efficacy on whether mechanical ventilation therapy is used in patients with acute exacerbation
