血清同型半胱氨酸 尿酸 脑钠肽前体及尿微量白蛋白联合检验在慢性肾小球肾炎辅助诊断中的应用观察

摘　　要：目的探讨血清同型半胱氨酸(HCY)、尿酸(UA)、脑钠肽前体(NT-proBNP)及尿微量白蛋白(U-mALB)联合检测在慢性肾小球肾炎(CGN)辅助诊断中的应用价值。方法选取2020年1月至2023年1月我院收治的60例CGN患者及60名健康体检者为观察组及对照组。均采血检测HCY、UA、NT-proBNP水平,并采尿检测U-mALB水平。比较2组HCY、UA、NT-proBNP及U-mALB水平差异,并比较不同分期患者各指标差异;绘制受试者工作特征(ROC)曲线分析各指标联合诊断CGN的临床价值。结果观察组HCY、UA、NT-proBNP及U-mALB水平为(19.0±1.5)μmol/L、(490±43)μmol/L、(446±52)pg/ml、(32.6±3.5)mg/L,高于对照组的(9.2±1.2)μmol/L、(306±30)μmol/L、(201±20)pg/ml、(14.1±1.2)mg/L(P<0.05);Ⅲ期CGN患者HCY、UA、NT-proBNP及U-mALB水平为(25.7±2.3)μmol/L、(543±50)μmol/L、(502±55)pg/ml、(40.5±4.2)mg/L,高于Ⅱ期、Ⅰ期患者(P<0.05)。绘制ROC曲线分析,HCY、UA、NT-proBNP、U-mALB及联合诊断CGN的AUC值为0.800、0.814、0.816、0.836、0.947,联合检测价值最高。结论血清HCY、UA、NT-proBNP及U-mALB在CGN诊断中具有一定价值,联合检测诊断价值最高,利于早期识别肾脏病变,改善患者预后。Objective To explore the value of serum homocysteine(HCY),uric acid(UA),N-terminal pro-brain natriuretic peptide(NT-proBNP)and urinary microalbumin(U-mALB)in the auxiliary diagnosis of chronic glomerulonephritis(CGN).Methods Sixty CGN patients and sixty healthy physical examination patients admitted to our hospital from January 2020 to January 2023 were selected as the observation group and the control group.All blood samples were collected for HCY,UA,and NT-proBNP levels,and urine collection for U-mALB levels.Comparing HCY,UA,NT-proBNP and U-mALB levels between the two groups,and the differences of CGN patients with different stages.ROC curve was used to analyze the clinical value of each index to diagnose CGN.Results The levels of HCY,UA,NT-proBNP and U-mALB in the observation group[(19.0±1.5)μmol/L,(490±43)μmol/L,(446±52)pg/ml,(32.6±3.5)mg/L],were higher than those in the control group[(9.2±1.2)μmol/L,(306±30)μmol/L,(201±20)pg/ml,(14.1±1.2)mg/L(P<0.05)];The levels of HCY,UA,NT-proBNP and U-mALB in stageⅢCGN patients[(25.7±2.3)μmol/L,(543±50)μmol/L,(502±55)pg/ml,(40.52±4.19)mg/L]were higher than those in stageⅡand stageⅠCGN patients(P<0.05).To draw the ROC curve analysis,the AUC values of HCY,UA,NT-proBNP,U-mALB and cooperative detection for diagnostic diagnosis were 0.800,0.814,0.816,0.836,0.947,the combined test has the highest value.Conclusion Serum HCY,UA,NT-proBNP and U-mALB have certain value in CGN diagnosis,with the highest combined detection and diagnostic value,which is conducive to the early identification of renal lesions and improvement of the prognosis of patients.
