12导联心电图与24 h动态心电图应用于起搏器植入患者检查中的价值对照研究

摘　　要：目的研究12导联心电图与24 h动态心电图应用于起搏器植入患者检查中的价值。方法以本院2022年3月至2024年3月110例起搏器植入患者为研究对象,所有患者均接受12导联心电图与24 h动态心电图检查,比较2种检查方式对起搏器功能异常、心肌缺血、心律失常及心率变异性时域指标变化的检出差异。结果24 h动态心电图起搏器功能异常检出率47.3%(52/110)高于12导联心电图8.2%(9/110)(χ^(2)=41.940,P<0.05);24 h动态心电图心肌缺血检出率26.4%(27/110)高于12导联心电图6.4%(7/110)(χ^(2)=16.075,P<0.05);24 h动态心电图心律失常检出率49.1%(54/110)高于12导联心电图9.1%(10/110)(χ^(2)=42.660,P<0.05);24 h动态心电图心率变异性时域指标均高于12导联心电图(P<0.001)。结论24 h动态心电图检查灵敏度较高,可有效检出起搏器植入患者起搏器功能异常、心肌缺血、心律失常及心率变异性时域指标变化情况,值得推广。Objective To study the value of 12-lead electrocardiogram and 24 h holter electrocardiogram in patients with pacemaker implantation.Methods A total of 110 patients with pacemaker implantation in our hospital from March 2022 to March 2024 were studied.All patients received 12-lead electrocardiogram(ECG)and 24 h holter electrocardiogram(HOLTER)examination,and the differences in the detection of pacemaker dysfun-ction,myocardial ischemia,arrhythmia and heart rate variability(HRVF)by the two examination methods were compared.Results The detection rate of abnormal function of 24 h holter ECG was 47.3%(52/110)higher than that of 12-lead ECG 8.2%(9/110)(χ^(2)=41.940,P<0.05).The detection rate of myocardial ischemia in 24 h holter ECG was 26.4%(29/110)higher than that in 12-lead ECG 6.4%(7/110)(χ^(2)=16.075,P<0.05).The detection rate of arrhythmia in 24 h holter ECG was 49.1%(54/110)higher than that in 12-lead ECG 9.1%(10/110)(χ^(2)=42.660,P<0.001).The time domain index of heart rate variability in 24 h holter ECG was higher than that in 12-lead ECG(P<0.001).Conclusion 24-hour Holter ECG examination has high sensitivity and can effectively detect the changes of pacemaker dysfunction,myocardial ischemia,arrhythmia and heart rate variability in patients with pacemaker implantation,which is worthy of promotion.
