专项质量管理对消毒供应中心软式腔镜消毒及灭菌质量的影响

摘　　要：目的探讨专项质量管理对消毒供应中心(CSSD)软式腔镜消毒及灭菌质量的影响。方法选取2022年1月至2023年12月在本院CSSD回收消毒的430件软式腔镜器械,以2022年1月至12月应用常规质量管理模式期间回收消毒的215件软式腔镜器械作为常规组,以2023年1月至2023年12月应用专项质量管理期间回收消毒的215件软式腔镜器械作为专项管理组。比较专项管理组和常规组软式腔镜器械的管理质量、管理合格率、腔镜器械的不良事件发生率、软式腔镜器械满意度及随访跟踪医院感染发生率。结果专项管理组清洗质量、消毒灭菌质量和包装质量评分均高于常规组(P<0.05);专项管理组清洗合格率、包装合格率、无菌物品合格率、发放合格率均高于常规组(P<0.05);专项管理组腔镜器械的不良事件发生率为0.47%(12/215)均低于常规组的3.72%(8/215)(χ^(2)=5.561,P=0.003);专项管理组服务态度、无菌物品及时供应、有效交流、无菌器械质量满意度评分均高于常规组;2组医院感染发生率比较差异无统计学意义(χ^(2)=1.307,P=0.753)。结论专项质量管理可有效提高对CSSD软式腔镜消毒、灭菌质量、器械管理合格率,有利于降低腔镜器械清洗、消毒、灭菌时的不良事件,各科室人员满意度较高。Objective To explore the impact of special quality management on the quality of disinfection and sterilization of soft luminal scopes in disinfection supply centre(CSSD).Methods A total of 430 flexible endoscopes recycled and disinfected in CSSD of the hospital were collected between January 2022 and December 2023,including 215 endoscopes in routine group(routine quality management,January-December 2022)and 215 endoscopes in special management group(special quality management,January-December 2023).The management quality of flexible endoscopes,qualification rate of management,incidence of endoscope-related adverse events,satisfaction with flexible endoscopes and incidence of nosocomial infection during follow-up were compared between the two groups..Results The scores of cleaning quality,sterilisation quality and packaging quality of the special management group were higher than those of the conventional group(P<0.05).The cleaning pass rate,packaging pass rate,sterile goods pass rate and issuance pass rate of the special management group were higher than those of the conventional group(P<0.05).The incidence rate of adverse events of the special management group′s luminaloscopic instruments was 0.47%(1/215)were lower than those of the conventional group 3.72%(8/215)(χ^(2)=5.561,P=0.003).The special management group′s service attitude,timely supply of sterile goods,effective communication,and satisfaction score of sterile instrument quality were higher than that of the conventional group(P<0.05).the incidence of hospital infection in the two groups was compared(χ^(2)=1.307,P=0.253).Conclusion Special quality management can effectively improve the quality of disinfection and sterilisation of CSSD soft luminal scopes,the qualification rate of instrument management,which is conducive to the reduction of adverse events during cleaning,disinfection and sterilisation of luminal scopes,and the satis-faction of the personnel of each department is high.
