摘 要:目的:探讨危重症新生儿万古霉素谷浓度监测达标率低的影响因素。方法:纳入2018年12月至2023年12月在西北妇女儿童医院接受万古霉素治疗的新生儿,接受静脉滴注万古霉素治疗的年龄≤28 d,胎龄≥26周妊娠出生的新生儿。分析万古霉素给药方案,监测稳态万古霉素谷浓度。结果:共纳入146例患儿,15.06%(22/146)的患儿万古霉素谷浓度<5 mg/L。单因素回归分析结果显示,胎龄、体质量、身高、万古霉素日剂量与万古霉素谷浓度显著相关(P<0.05)。多元线性回归分析显示,胎龄、体质量、万古霉素日剂量是万古霉素谷浓度低的独立影响因素(P<0.05)。结论:新生儿万古霉素谷浓度监测达标率低,尤其是重症新生儿,万古霉素日剂量、胎龄、体质量是重要的影响因素。Objective:To probe into the influencing factors of low compliance rate of vancomycin trough concentration monitoring in critically ill neonates.Methods:Neonates with vancomycin treatment admitted into the Northwest Women’s and Children’s Hospital from Dec.2018 to Dec.2023,aged≤28 d treated with intravenous vancomycin,and with gestational age≥26 weeks were enrolled.Results:A total of 146 patients were included,and 15.06%(22/146)of patients had a trough concentration of vancomycin<5 mg/L.Univariate regression analysis showed that gestational age,body weight,height,vancomycin daily dose were significantly correlated with trough concentration of vancomycin(P<0.05).Multiple linear regression analysis showed that gestational age,body weight,and vancomycin daily dose were independent influencing factors for low trough concentration of vancomycin(P<0.05).Conclusion:The daily dose,gestational age and body weight of vancomycin are the important influencing factors affecting the compliance rate of trough concentration of vancomycin monitoring in neonates,especially in severe neonates.
