依托孕烯植入剂对子宫内膜异位症或子宫腺肌病患者月经量和盆腔痛的影响

摘　　要：目的:评估依托孕烯植入剂治疗子宫内膜异位症(EMS)或子宫腺肌病且无生育要求患者月经量和盆腔痛效果。方法:纳入2019年9月-2023年10月本院就诊的EMS或子宫腺肌病患者85例,均使用依托孕烯植入剂,对于月经量、盆腔痛、月经出血模式、体重增加、乳房疼痛和任何其他治疗副作用进行随访1年。结果:85例中80例完成了12个月随访(94.1%),月经量减少占比(37.5%)低于放置前,甚至出现闭经(35.0%);视觉模拟量表(VAS)疼痛评分盆腔痛(0.56±0.12分)比放置前(6.17±2.75分)下降(P<0.05)。患者均未出现意外妊娠,避孕效果达到100%。部分患者治疗后出现体重增加、乳房胀痛、痤疮、皮肤瘙痒、性欲减退、情绪改变等不良反应。结论:依托孕烯植入剂可有效减少EMS或子宫腺肌病患者的月经量,减轻盆腔痛,同时兼具良好的避孕效果。Objective:To evaluate the effect of etonogestrel implant for treating patients with endometriosis or adenomyosis and without fertility requirements on their menstrual volume and pelvic pain.Methods:85 patients with endometriosis or adenomyosis who admitted in the hospital from September 2019 to October 2023 were included in this study.These patients were all given treatment of etonogestrel implant.The menstrual volume,the pelvic pain,the menstrual bleeding patterns,the weight gain,the breast pain and any the other treatment side effects of the patients were followed up for 1 year.Results:The proportion of the decreased menstrual volume(37.5%)of the patients after etonogestrel implant used was significantly lower than that(35.0%)before etonogestrel implant used,and the amenorrhea of 35.0%patients even occurred after etonogestrel implant used.The score of pelvic pain(0.56±0.12 points)evaluated by visual analogue scale(VAS)of the patients after etonogestrel implant used was significantly lower than that(6.17±2.75 points)before etonogestrel implant used(P<0.05).There was no any patient with the unwanted pregnancy or no any patient with 100%contraceptive effect.Some patients had the adverse reactions,such as weight gain,breast tenderness,acne,skin itching,decreased libido and mood changes after treatment.Conclusion:Etonogestrel implant for treating the patients with endometriosis or adenomyosis can effectively reduce their menstrual volume and relieve their pelvic pain,and which has good contraceptive effect.
