耳穴压豆联合穴位贴压法对剖宫产术后疼痛、炎性反应、焦虑及血清BDNF、Cor、IL-6、TNF-α的影响

摘　　要：目的:探讨耳穴压豆联合穴位贴压法对剖宫产术后疼痛、炎性反应、焦虑及血清脑源性神经营养因子(BDNF)、血清皮质醇(Cor)、血清白细胞介素-6(IL-6)、血清肿瘤坏死因子-α(TNF-α)水平的影响。方法:选取2022年6月-2024年6月本院剖宫产妇女60例,采用随机数字表法分为观察组(n=30例)和对照组(n=30例)。对照组予常规术后治疗,观察组在常规术后治疗基础上,予采用耳穴压豆联合穴位贴压法治疗。结果:术后24、48h时,观察组SAS评分低于对照组(t=4.98,3.05;P<0.05);术后6、12、24、48h宫缩、静息、咳嗽时VAS疼痛评分观察组低于对照组(P<0.05),观察组镇痛泵按压次数明显减少[(4.1±1.0)次比(5.5±1.1)次;t=4.98,P<0.001];IL-6、TNF-α、Cor水平观察组低于对照组(t=10.74,6.11,2.53;P<0.05);BDNF水平观察组高于对照组(t=6.70,P<0.05)。结论:耳穴压豆联合穴位贴压法,可缓解剖宫产术后疼痛、炎性反应及患者焦虑程度。Objective:To investigate the effects of the ear acupoint pressing bean and the acupoint pressing for women after cesarean section on their pain situation,inflammatory response,anxiety,and the levels of their serum brain-derived neurotrophic factor(BDNF),cortisol(Cor),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α).Methods:A total of 60 pregnant women who wanted the cesarean section were selected and were randomly divided into two groups(30 cases in each group)by random number table method from June 2022 to June 2024.The women in the control group were given the routine postoperative treatment after cesarean section,while the women in the study group were treated with the ear acupoint pressing bean and the acupoint pressing on the basis of the routine postoperative treatment.Results:The HSAS scores of the women in the study group at 24h and at 48h after cesarean section were significantly lower than those of the women in the control group(t=4.98,3.05,P<0.05).The VAS pain scores of the women in the study group at uterine contraction,at rest and at cough in 6,12,24 and 48h after cesarean section were significantly lower than those of the women in the control group(P<o.05).The number of analgesic pump pressing(4.1±1.0 times)of the women in the study group was significantly less than that(5.5±1.1 times)of the women in the control group(t=4.98,P<0.001).The levels of IL-6,TNF-α and Cor of the women in the study group were significantly lower than those of the women in the control group(t=10.74,6.11,2.53,P<0.05).The BDNF level of the women in the study group was significantly higher than that of the women in the control group(t=6.70,P<0.05).Conclusion:The ear acupoint pressing bean and the acupoint pressing for the women after cesarean section can relieve their postoperative pain,inflammatory response and anxiety.
