摘  要:目的:考察新生儿院外转运对极低出生体重儿(VLBWI)的预后效果。方法:收集2021年1月-2022年12月采取新生儿院外转运方式入住本院NICU的VLBWI 85例(转运组)、非新生儿院外转运方式入住本院新生儿重症监护(NICU)的VLBWI 60例(非转运组),分析VLBWI进入NICU 5min内体温、低体温发生、达到正常体温时间、进入NICU接受呼吸机治疗情况、进入NICU治疗时间、并发症发生情况及预后、随访6个月发育状况。结果:两组进入NICU低体温发生、接受呼吸机治疗占比均无差异(均P>0.05),非转运组进入NICU 5min体温(37.02±0.86℃)低于转运组(37.35±0.95℃),达到正常体温时间(7.25±1.98d)长于转运组(5.61±0.56d),呼吸机治疗时间(11.39±2.88h)长于转运组(7.21±1.69h),住院时间(7.25±1.98d)长于转运组(5.61±0.56d),并发症(91.7%)高于转运组(76.5%)(均P<0.05);死亡率(5.2%)与转运组(2.3%)无差异(P>0.05),死因为呼吸窘迫综合征、感染、脑出血,死亡时间为出生1~10d。随访6~10个月,随访中位数为7.5个月,两组均出现发育迟缓、神经发育问题及其他等发育异常,非转运组VLBWI发育正常占比(63.2%)低于转运组(79.5%)(P<0.05)。结论:新生儿院外转运可有效改善预后。Objective:To investigate the prognosis of very low birth weight infants(VLBWI)after the neonatal extramural transport.Methods:85 cases of VLBWI admitted to the neonatal intensive care unit(NICU)after the neonatal extramural transport were collected in study group from January 2021 to December 2022,and 60 cases of VLBWI admitted to the NICU who had not been given the neonatal extramural transport were collected in control group during the same period.The body temperature and the rate of hypothermia within 5 minutes of admission in NICU,the time to reach the normal body temperature,the ventilator treatment situation,the NICU admission time,the complications rate,the prognosis and the development status during 6-month of follow-up of the infants in the two groups were recorded.Results:There were no significant differences in the incidences of the hypothermia and ventilator treatment of the infants after admission to NICU between the two groups(all P>0.05).The body temperature(37.02±0.86℃)at 5 minutes after admission to NICU of the infants in the control group was significantly lower than that(37.35±0.95℃)of the infants in the study group.The time of reaching the normal body temperature(7.25±1.98d)of the infants in the control group was significantly longer than that(5.61±0.56d)of the infants in the study group.The ventilator treatment time(11.39±2.88h)of the infants in the control group was significantly longer than that(7.21±1.69 h)of the infants in the study group.The duration of the hospitalization(7.25±1.98d)of the infants in the control group was significantly longer than that(5.61±0.56d)of the infants in the study group,and the incidence of the complications(91.7%)of the infants in the control group was significantly higher than that(76.5%)of the infants in the study group(all P<0.05).There was no significant difference in the mortality(5.2%vs.2.3%)of the infants between the two groups(P>0.05).The causes of the death of the infants in the two groups were respiratory distress syndrome,infection and
