经阴道超声参数在输卵管妊娠诊断及保守治疗后卵巢储备功能恢复中的评估效果

摘　　要：目的:分析经阴道超声(TVS)参数在输卵管妊娠诊断及保守治疗后卵巢储备功能恢复中的评估效果。方法:回顾性选取2022年9月-2023年9月本院收治的临床怀疑输卵管妊娠者88例临床资料,均经阴道超声检查,以临床最终诊断结果为标准,明确诊断为输卵管妊娠者给予保守治疗,并按治疗效果分为治疗成功组(n=66)和治疗失败组(n=13),分析TVS诊断输卵管妊娠阳性情况,比较输卵管妊娠患者患侧与健侧卵巢动脉超声参数[阻力指数(RI)、搏动指数(PI)和峰值流速(PSV)],比较治疗成功组与治疗失败组卵巢动脉超声参数及卵巢储备功能恢复情况。结果:88例中临床最终诊断为输卵管妊娠阳性79例,TVS显示为输卵管妊娠阳性78例,TVS诊断输卵管妊娠的敏感性98.7%、特异性100.0%及准确率98.9%。输卵管妊娠患者患侧卵巢动脉RI、PI值均低于健侧,PSV值高于健侧;治疗成功组卵巢动脉RI(0.78±0.16)、PI(2.81±0.55)值均高于治疗失败组(0.63±0.13、2.39±0.47),PSV值(10.35±2.07 cm/s)低于治疗失败组(12.81±2.56 cm/s),卵巢体积(5.25±1.06)cm3和窦卵泡数目(4.78±0.91个)均高于治疗失败组(4.51±0.92)cm^(3)、(4.13±0.77个)(均P<0.05)。结论:TVS参数可反映病灶滋养动脉血流异常情况,对早期诊断输卵管妊娠及保守治疗后评估卵巢储备功能恢复情况均具有重要临床价值。Objective:To analyze the assessment effect of the transvaginal sonography(TVS)parameters for the diagnosis of tubal pregnancy and the ovarian reserve function recovery of patients after the conservative treatment.Methods:The clinical data of 88 patients with the suspected tubal pregnancy who had admitted to the hospital from September 2022 to September 2023 were selected in this study retrospectively.These patients had received TVS.The results of the clinical final diagnosis of the patients were as the standard,the patients with the confirmed tubal pregnancy were given the conservative treatment and were divided into group A(66 patients with the treatment success)and group B(13 patients with the treatment failure)according to the therapeutic effect of the patients.The positive status of the tubal pregnancy of the patients diagnosed by TVS was analyzed.The values of the ultrasound parameters of the ovarian artery,such as resistance index(RI),pulsatility index(PI)and peak systolic velocity(PSV),of the patients with tubal pregnancy were compared between the healthy side of the fallopian tube and the pregnancy side of the fallopian tube.The values of the ultrasound parameters of the ovarian artery and the recovery of the ovarian reserve function of the patients were compared between group A and group B.Results:Among 88 patients,there were 79 cases with the clinically diagnosed as the tubal pregnancy finally.78 patients were diagnosed as the tubal pregnancy by TVS,with the sensitivity of 98.7%,the specificity of 100.0%and the accuracy of 98.9%.The RI and PI values of the ovarian artery of the fallopian tube healthy side of the patients were significantly lower than those of the fallopian tube pregnancy side.The values of RI(0.78±0.16)and PI(2.81±0.55)of the ovarian artery of the patients in group A were significantly higher than those(0.63±0.13 and 2.39±0.47)of the patients in group B.The PSV value(10.35±2.07 cm/s)of the ovarian artery of the patients in group A was significantly lower than that(12.81±2.56 cm/s)
