呼吸窘迫综合征早产儿血清BMP-7、CC-16水平与SNAPPE-Ⅱ评分的相关性

摘  要:目的:分析呼吸窘迫综合征早产儿血清骨形态发生蛋白-7(BMP-7)、Clara细胞分泌蛋白-16(CC-16)水平与新生儿急性生理学评分围产期补充Ⅱ(SNAPPE-Ⅱ)评分的相关性及预后患儿价值。方法:回顾性收集2019年3月-2023年3月本院出生的呼吸窘迫综合征早产儿95例临床资料,依据其预后情况分为预后良好组(n=71)与预后不良组(n=24)。收集患儿血清BMP-7、CC-16水平,评价SNAPPE-Ⅱ评分。采用Pearson相关性分析血清BMP-7、CC-16水平与SNAPPE-Ⅱ评分的相关性,采用Spearman秩相关性分析血清BMP-7,CC-16水平、SNAPPE-Ⅱ评分与早产儿呼吸窘迫综合征预后的相关性,以受试者工作特征(ROC)曲线评价血清BMP-7,CC-16水平、SNAPPE-Ⅱ评分对患儿预后的预测价值。结果:预后不良组血清BMP-7(47.63±10.64 ng/L)、CC-16(47.44±11.86 ng/L)、SNAPPE-Ⅱ评分(30.33±8.85分)均高于预后良好组(41.24±8.42 ng/L、36.97±8.56 ng/L、17.75±4.76分)(均P<0.05);相关性分析显示,血清BMP-7、CC-16水平与SNAPPE-Ⅱ评分呈正相关(r=0.319、0.335,均P<0.05),血清BMP-7、CC-16、SNAPPE-Ⅱ评分与预后不良均呈正相关(r=0.280、0.378、0.588,均P<0.05);ROC曲线分析,血清BMP-7、CC-16、SNAPPE-Ⅱ评分联合预测呼吸窘迫综合征患儿预后的曲线下面积0.959,灵敏度95.8%,特异性93.0%,预测效能优于各指标单独评估(均P<0.05)。结论:呼吸窘迫综合征早产儿血清BMP-7、CC-16与SNAPPE-Ⅱ评分密切相关,通过早期3项指标联合评估早产儿预后有较高临床价值。Objective:To analyze the correlation between the levels of serum bone morphogenetic protein-7(BMP-7)and Clara cell protein-16(CC-16)of premature infants with respiratory distress syndrome(RDS)and their supplement of neonatal acute physiology perinatalⅡ(SNAPPE-Ⅱ)score,and to study the values of the serum BMP-7 and CC-16 levels of the premature infants with RDS for predicting their prognosis.Methods:The clinical data of 95 premature infants with RDS who were born in the hospital from March 2019 to March 2023 were collected retrospectively.These infants were divided into group A(71 cases with the good prognosis)and group B(24 cases with the poor prognosis)according to the prognosis status of the infants.The levels of the serum BMP-7 and CC-16 of the infants in the two groups were detected,and the SNAPPE-Ⅱscore of the infants in the two groups was evaluated.Pearson correlation analysis was used to analyze the correlation between the serum BMP-7 and CC-16 levels of the preterm infants with RDS and their SNAPPE-Ⅱscore.Spearman rank correlation analysis was used to analyze the correlation between the serum BMP-7 and CC-16 levels of the preterm infants with RDS and their prognosis.Receiver operating characteristic(ROC)curve was used to evaluate the predictive value of the serum BMP-7 and CC-16 levels and the SNAPP-Ⅱscore of the preterm infants with RDS for their prognosis.Results:The levels of the serum BMP-7(47.63±10.64 ng/L)and CC-16(47.44±11.86 ng/L),and the SNAPPE-Ⅱ score(30.33±8.85 points)of the infants in group B were significantly higher than those(41.24±8.42 ng/L,36.97±8.56 ng/L and 17.75±4.76 points)of the infants in group A(all P<0.05).The correlation analysis showed that the serum BMP-7 and CC-16 levels of the preterm infants with RDS were positively correlated with their SNAPPE-Ⅱ score(r=0.319,0.335,all P<0.05),and the serum BMP-7 and CC-16 levels,and the SNAPPE-Ⅱ score of the preterm infants with RDS were positively correlated with poor prognosis(r=0.280,0.378,0.588,all P<0.05).ROC curv
