幽门螺杆菌相关胃癌组织miRNA-19a、miRNA-146a与患者临床参数及远期生存的关系

摘　　要：[目的]研究幽门螺杆菌(Helicobacter pylori,Hp)相关胃癌组织miRNA-19a、miRNA-146a与患者临床参数及远期生存的关系。[方法]选取入院行手术切除的胃癌患者100例。根据患者的Warthin-Starry银染色结果将患者分为Hp阳性组(60例)和Hp阴性组(40例)。采用实时荧光定量法(qRT-PCR)测定患者胃癌组织和癌旁组织中miRNA-19a、miRNA-146a相对表达水平,比较胃癌组织和癌旁组织miRNA-19a、miRNA-146a相对表达水平。采用Pearson相关性分析法分析不同胃癌组织miRNA-19a、miRNA-146a表达相关性,比较不同临床病理特征Hp阳性胃癌患者癌组织miRNA-19a、miRNA-146a相对表达量。分析miRNA-19a、miRNA-146a表达与Hp阳性胃癌患者生存预后的关系,采用COX多因素分析法分析Hp阳性胃癌患者预后的危险因素。[结果]患者癌组织中miRNA-19a相对表达水平均显著高于癌旁组织(P<0.05),且Hp阳性组miRNA-19a相对表达水平均显著高于Hp阴性组(P<0.05)。患者癌组织中miRNA-146a相对表达水平均显著低于癌旁组织(P<0.05);Hp阳性组与Hp阴性组的癌旁组织中miRNA-19a相对表达水平比较差异无统计学意义(P>0.05)。Hp阳性组和Hp阴性组的胃癌组织中miRNA-19a和miRNA-146a的相对表达水平无相关性(r=-0.063,P=0.573;r=-0.124,P=0.387)。不同肿瘤直径、淋巴结转移及远处转移的Hp阳性患者癌组织miRNA-19a、miRNA-146a相对表达量组间比较均差异有统计学意义(P<0.05)。miRNA-19a低表达、高表达患者术后5年的生存率分别为48.39%、20.69%,组间比较差异有统计学意义(χ2=5.053,P=0.023)。miRNA-146a高表达、低表达患者术后5年的生存率分别为46.67%、20.00%,组间比较差异有统计学意义(χ2=4.800,P=0.0285)。COX回归分析结果显示,有远处转移、较高的TNM分期、miRNA-19a高表达及miRNA-146a低表达均是Hp阳性胃癌患者预后的独立危险因素(P<0.05)。[结论]Hp阳性患者胃癌组织miRNA-19a呈高表达,miRNA-146a呈低表达,与患者�[Objective]To study the relationship between miRNA-19a,miRNA-146aand clinical parameters and long-term survival of patients with Helicobacterpylori(Hp)-related gastric cancer.[Methods]A total of 100patients with gastric cancer who were admitted for surgical resection were selected.According to the Warthin-Starry silver staining results,the patients were divided into Hppositive gastric cancer patients and Hpnegative gastric cancer patients,with 60cases and 40cases respectively.The relative expression levels of miRNA-19aand miRNA-146ain gastric cancer tissues and adjacent tissues were measured by real-time fluorescence quantitative method(qRT-PCR).The relative expression levels of miRNA-19aand miRNA-146ain gastric cancer and adjacent tissues were compared.Pearson correlation analysis was used to analyze the expression correlation of miRNA-19aand miRNA-146ain different gastric cancer tissues.The relative expression levels of miRNA-19aand miRNA-146ain cancer tissues of Hp-positive gastric cancer patients with different clinicopathological features were compared.The relationship between the expression of miRNA-19aand miRNA-146aand the survival and prognosis of Hp-positive gastric cancer patients were analyzed.COX multivariate analysis was used to analyze the risk factors of prognosis of Hppositive gastric cancer patients.[Results]The relative expression level of miRNA-19ain cancer tissues was significantly higher than that in adjacent tissues(P<0.05),and the relative expression level of miRNA-19ain Hppositive patients was significantly higher than that in Hpnegative patients(P<0.05).The relative expression level of miRNA-146ain cancer tissues was significantly lower than that in adjacent tissues(P<0.05).Compared with Hp-negative patients,the relative expression level of miRNA-19ain adjacent tissues of Hp-positive patients was not statistically significant(P>0.05).The relative expression levels of miRNA-SSSSSST19aand miRNA-146ain gastric cancer tissues of Hppositive and Hpnegative patients had no correlation(r=-0.063,P
