透明帽辅助内镜下聚桂醇注射液剂型对伴有出血的内痔疗效影响

摘　　要：[目的]探讨透明帽辅助内镜下聚桂醇注射液剂型对伴有出血的内痔疗效的影响。[方法]选择36例伴有出血的Ⅰ~Ⅲ度内痔患者进行透明帽辅助内镜下内痔硬化治疗。12例患者行硬化剂原液治疗(LS组),24例患者行泡沫硬化剂治疗(FS组)。[结果]LS组与FS组患者共进行39例次(LS组13例次、FS组26例次)操作。2组患者的性别、年龄、内痔分度、并发症比较均差异无统计学意义。2组患者反转倒镜下的痔核数量、顺镜下的痔核数量、反转倒镜下的注射点数、顺镜下的注射点数比较均差异无统计学意义。与LS组患者比较,FS组患者每例注射硬化剂总量、每例每点注射硬化剂总量、反转倒镜下每点注射硬化剂量、顺镜下每点注射硬化剂量均显著减少。2组患者术后疼痛、术后12周随访评价治疗的有效性和满意率比较均差异无统计学意义。[结论]透明帽辅助内镜下泡沫硬化剂注射能够治疗伴有出血的Ⅰ~Ⅲ度内痔,具有硬化剂使用量更少、住院费用更低的优点。[Objective]To study the therapeutic effect of dose form of lauromacrogol injection on internal hemorrhoids accompanying hemorrhage.[Methods]Cap-assisted endoscopic sclerotherapy(CAES)was performed to the consecutive inpatients with grade I-III internal hemorrhoids accompanying hemorrhage.A total of 36patients were participated in the study with 12patients in the liquid sclerotherapy(LS)group,and then 24patients in the foam sclerotherapy(FS)group.[Results]Operation was performed 39times(LS group 13cases,FS group 26cases)in all the inpatients.There were no statistically significant differences in the gender,age,grade of hemorrhoids and postoperative complications,in the hemorrhoidal nucleus and injective numbers,and in postoperative transient pain score,effectivity and satisfaction during the 12-week follow-up between LS group and FS group.However,there were highly significant differences in total sclerosing amount per patient,total sclerosing amount per spot between the two groups.[Conclusion]There is less sclerosing amount if foam sclerotherapy is used for treating grade I-III internal hemorrhoids accompanying hemorrhage by CAES.The foam sclerotherapy has the advantage of less hospitalization costs,and maybe has lower incidence of postoperative pain and complications and more effectivity and satisfaction rates if our studies are further carried out.
