摘 要:目的:系统评价输尿管镜碎石手术病人术后疼痛的危险因素。方法:检索中国知网、万方数据库、维普数据库、SinoMed、Web of Science、PubMed、Embase、the Cochrane Library中有关输尿管镜碎石手术病人术后疼痛危险因素的研究,检索时限为建库至2023年9月26日。通过纽卡斯尔⁃渥太华量表(NOS)评价文献质量,采用RevMan 5.4软件完成数据分析。结果:留置输尿管支架管[OR=5.09,95%CI(2.59,10.00),P<0.0001]、24 cm双J管[OR=10.82,95%CI(4.48,26.15),P<0.00001]、26 cm双J管[OR=11.36,95%CI(4.60,28.05),P<0.00001]、结石残留[OR=2.92,95%CI(1.69,5.06),P<0.0001]、尿路感染[OR=2.75,95%CI(1.40,5.42),P=0.003]是输尿管镜碎石手术病人术后疼痛的危险因素。结论:诸多因素影响输尿管镜碎石手术病人的术后疼痛,提高对疼痛的认识更有利于其防控,亟需大样本、多中心的研究全面探索输尿管镜碎石术后疼痛的危险因素并构建预测模型。Objective:To systematically evaluate the risk factors for postoperative pain in patients undergoing ureteroscopic lithotripsy.Methods:Studies on risk factors for postoperative pain in patients undergoing ureteroscopic lithotripsy were retrieved from CNKI,Wanfang Database,VIP Database,SinoMed,Web of Science,PubMed,Embase,and the Cochrane Library,with the search period spanning from inception to September 26,2023.Literature quality was assessed using the Newcastle⁃Ottawa Scale(NOS),and data analysis was performed using RevMan 5.4 software.Results:Indwelling ureteral stent[OR=5.09,95%CI(2.59,10.00),P<0.0001],24 cm double⁃J stent[OR=10.82,95%CI(4.48,26.15),P<0.00001],26 cm double⁃J stent[OR=11.36,95%CI(4.60,28.05),P<0.00001],residual stones[OR=2.92,95%CI(1.69,5.06),P<0.0001],and urinary tract infection[OR=2.75,95%CI(1.40,5.42),P=0.003]were identified as risk factors for postoperative pain in patients undergoing ureteroscopic lithotripsy.Conclusions:Multiple factors influence postoperative pain in patients undergoing ureteroscopic lithotripsy.Enhanced awareness of pain is more conducive to its prevention and control.There is an urgent need for large⁃sample,multicenter studies to comprehensively explore the risk factors for postoperative pain following ureteroscopic lithotripsy and to develop predictive models.
