基于国家区域医疗中心的三级管理模式在中心静脉导管维护资质培训中的应用

摘　　要：目的:探讨国家区域医疗中心建设背景下的三级管理模式在中心静脉导管维护资质培训中的应用效果。方法:选取2022年7月—9月参加华北地区某三级甲等医院中心静脉导管维护资质培训的68名护士进行培训,调查其一般人口学资料、培训反馈、工作情况、培训前后工作效果。结果:培训反馈总分为109分,经过培训的护士返回岗位半年后成为科室静脉治疗护理骨干的占55.88%、工作内容有变化的占2.94%,培训后的理论及操作成绩明显高于培训前的成绩(P<0.05),培训后的导管维护合格率明显高于培训前(P<0.05),导管相关血流感染发生率明显降低(P<0.05)。结论:基于国家区域医疗中心的三级管理模式在中心静脉导管维护资质培训中的应用效果显著,提高了导管维护合格率,降低了导管相关血流感染发生率,促进了华北地区中心静脉治疗专科护士培养及专科发展,为加强中心静脉导管维护资质护士的培训与管理提供参考。Objective:To explore the application effect of a three⁃level management model under the framework of the national regional medical center in qualification training for central venous catheter(CVC)maintenance.Methods:A total of 68 nurses participating in CVC maintenance qualification training at a tertiary Grade A hospital in North China from July to September 2022 were enrolled.Data on general demographics,training feedback,work status,and pre⁃and post⁃training work effectiveness were collected and analyzed.Results:The total training feedback score was 109 points.Six months post⁃training,55.88%of the nurses became core members of venous therapy nursing in their departments,while 2.94%reported changes in their job responsibilities.Post⁃training theoretical and operational scores were significantly higher than pre⁃training scores(P<0.05).The post⁃training CVC maintenance compliance rate significantly increased(P<0.05),and the incidence of catheter⁃related bloodstream infections significantly decreased(P<0.05).Conclusions:The three⁃level management model based on the national regional medical center demonstrated significant effectiveness in CVC maintenance qualification training.It improved compliance rates for catheter maintenance,reduced catheter⁃related bloodstream infections incidence,and facilitated the development of specialized venous therapy nursing in North China.This study provides valuable insights for enhancing the training and management of nurses qualified in CVC maintenance.
