摘　　要：目的:通过系统的检索、评价以及总结重症俯卧位机械通气病人体位管理的相关证据,为重症医学科医护人员实施俯卧位体位管理提供依据。方法:按照文献质量自上而下在国内外数据库、网站中检索临床决策、最佳实践、指南、专家共识、系统评价、Meta分析、证据总结,检索时限为2012年1月1日—2023年5月24日。选取合适的文献评价工具,过程由研究者独立进行筛选。结果:最终纳入14篇文献,其中临床决策2篇,指南5篇、系统评价1篇、专家共识4篇,最佳证据总结2篇。从支撑面使用、预防性敷料使用、体位摆放处理3个方面进行汇总,总结了20条重症俯卧位机械通气病人体位管理的最佳证据。结论:本研究操作性强且具有临床指导意义,在整合证据的基础上,为制订临床管理方案提供科学依据,促进护理实践的进步。Objective:To systematically retrieve,evaluate,and synthesize evidence on positioning management for critically ill patients undergoing prone position mechanical ventilation,providing a basis for healthcare professionals in intensive care units to implement prone positioning strategies.Methods:A top⁃down literature quality assessment was conducted by searching databases and websites for clinical decision⁃making articles,best practices,guidelines,expert consensuses,systematic reviews,Meta⁃analyses,and evidence summaries published from January 1,2012,to May 24,2023.Appropriate literature evaluation tools were selected,and the screening process was independently performed by researchers.Results:A total of 14 articles were included,comprising 2 clinical decision⁃making articles,5 guidelines,1 systematic review,4 expert consensuses,and 2 evidence summaries.The evidence was synthesized into three categories,support surface usage,prophylactic dressing application,and positioning handling.A total of 20 best evidence statements for positioning management in critically ill patients undergoing prone position mechanical ventilation were summarized.Conclusions:The findings are highly operational and clinically significant,offering a scientific foundation for developing clinical management protocols and advancing nursing practice.
