摘 要:目的对比基弧5.6 mm和6.2 mm的角膜塑形镜对近视的控制效果。方法前瞻性队列研究。纳入濮阳市眼科医院眼视光门诊2022年1月至2023年6月儿童青少年近视115例。其中男55例,女60例;年龄(10.36±1.87)岁,范围8~14岁。受试者分为两组,佩戴基弧5.6 mm组为观察组,基弧6.2 mm组为对照组。对比两组戴镜后3个月光学治疗区直径和角膜点染情况,及戴镜后1年眼轴长度变化。结果戴镜后3个月,观察组和对照组光学治疗区直径分别为(3.17±0.39)、(3.50±0.27)mm,两组差异具有统计学意义(t=-5.21,P<0.001);两组角膜染色分级差异无统计学意义(χ^(2)=0.52,P=0.474)。戴镜1年后,观察组和对照组眼轴差值(戴镜后减去戴镜前)分别为(0.17±0.12)和(0.33±0.34)mm,两组差异具有统计学意义(t=2.89,P=0.005)。结论相较于基弧6.2 mm角膜塑形镜,配戴基弧5.6 mm角膜塑形镜后3个月的光学治疗区直径更小,1年的眼轴增长更少,近视防控效果更优。Objective To compare the effects of 5.6 mm and 6.2 mm base curve orthokeratology lenses in the prevention and control of myopia.Methods This was a prospective cohort study.A total of 115 children with myopia in Puyang Eye Hospital from Jan.2022 to Jun.2023 were included.There were 55 males and 60 females.The age was(10.36±1.87)years old,ranged from 8 to 14 years.The subjects were divided into two groups based on different base curves.Subjects of the observation group were wearing 5.6 mm base curve orthokeratology lenses and the control group were wearing 6.2 mm.Optic treatment zone diameter(TZD)and corneal staining were compared after wearing lenses for 3 months.Changes in axial length(AL)were compared after wearing lenses for 1 year.Results After wearing orthokeratology lenses for 3 months,TZD of the observation group and the control group were(3.17±0.39)and(3.50±0.27)mm,respectively.There was statistical significance in TZD,while there was no statistical difference in corneal staining(χ^(2)=0.52,P=0.474).Changes of AL(after-before wearing orthokeratology lenses)at 1 year in the observation group[(0.17±0.12)mm]were significantly different from those in the control group[(0.33±0.34)mm](t=2.89,P=0.005).Conclusion Compared with 6.2 mm base curve orthokeratology lenses,5.6 mm base curve orthokeratology lenses produce a smaller TZD after 3 months and slower growth of axial length after 1 year.The effect of 5.6 mm base curve orthokeratology lenses in the prevention and control of myopia is better than that of the 6.2 mm ones.
