神经松动术联合麦肯基疗法治疗腰椎间盘突出症的疗效

摘　　要：目的观察神经松动术联合麦肯基疗法治疗腰椎间盘突出症(LDH)的临床效果。方法选取我院康复医学科收治的LDH患者70例,根据患者入院的先后顺序将其分为观察组和对照组,每组35例。对照组采用药物治疗、常规物理治疗、中医康复技术;观察组在常规治疗的基础上采用神经松动术联合麦肯基疗法。两组均持续治疗3个月。比较两组患者治疗前后视觉疼痛模拟评分(VAS)、腰椎活动度、腰椎功能评分(JOA)、SF-36生活质量表评分。结果治疗后观察组患者VAS评分低于对照组(P<0.05);腰椎前屈、后伸、侧屈活动度均高于对照组(P<0.001);腰椎功能(JOA)评分高于对照组(P<0.001);SF-36生活质量评分各个维度均高于对照组(P<0.001)。结论对LDH患者实施神经松动术+麦肯基疗法康复干预,可有效地恢复腰椎功能,缓解腰腿疼痛,增加腰椎的活动范围和协调性,提高生活质量。Objective To observe the clinical effect of neuroloosening combined with McKenzie therapy for lumbar disc herniation(LDH).Methods 70 LDH patients in our hospital were selected and randomly divided into the control group and the observation group,with 35 cases in each group.The control group was treated with medication,conventional physical therapy,and traditional Chinese medicine rehabilitation techniques.In the observation group,neuroloosening combined with McKenzie therapy was adopted on the basis of the ontrol group.Both groups were treated for 3 months.The visual pain simulation score(VAS),lumbar spine mobility,lumbar spine function score(JOA),SF-36 health survey short form score before and after treatment were compared between the two groups.Results After treatment,the VAS scores of the observation group were lower than those of the control group(P<0.05);the lumbar anterior flexion,posterior extension and lateral flexion mobility were higher than those of the control group(P<0.001);lumbar spine function(JOA)scores were higher than those in the control group(P<0.001);the SF-36 quality of life scores were higher than that in the control group(P<0.001).Conclusion Neuroloosening combined with McKenzie therapy can effectively restore lumbar spine function,alleviate lumbar and leg pain,increase the range and coordination of the lumbar spine motion,and improve the quality of life in rehabilitation of lumbar disc herniation.
