地塞米松和医用蜂蜜对下颌阻生第三磨牙拔除术后的临床疗效观察

摘　　要：目的:探讨地塞米松和医用蜂蜜对下颌阻生第三磨牙拔除术后炎性并发症的疗效。方法:选取下颌阻生智齿拔除术病人80例,随机分为4组,对照组、地塞米松组、医用蜂蜜组、地塞米松和医用蜂蜜联用组,各20例,术后第1、3、7天随访,比较4组病人术后切牙间距(大张口)、疼痛程度、使用的镇痛剂总量和面部肿胀。结果:术后同一时间地塞米松组、医用蜂蜜组和联用组与对照组相比,切牙间距较小、疼痛程度较低、使用的镇痛剂总剂量较少、面部肿胀较轻,差异均有统计学意义(P<0.05);除术后第1天,联用组与地塞米松组切牙间距差异无统计学意义外(P>0.05),其余各指标术后同一时间联用组与地塞米松组或医用蜂蜜组比较,联用组效果更好(P<0.05~P<0.01)。结论:术前使用地塞米松或术后使用医用蜂蜜可以减少下颌阻生第三磨牙拔除术后炎性并发症,两药单用或联用均可提供简单、安全、无痛的方法来消除术后不适,且两药联用效果更好。Objective:To investigate the effect of dexamethasone and medical-honey in the inflammatory complications after mandibular impacted third molar extraction.Methods:Eighty eligible patients were selected and randomly divided into the control group,dexamethasone group,medical-honey group,and combined group(dexamethasone and medical-honey),with 20 cases in each group.The patients were followed up at 1 day,3 days and 7 days after surgery,and the postoperative incisor distance in the state of wide-open mouth,visual analogue scale,total analgesic dose,and facial swelling were compared among the four groups.Results:The dexamethasone group,medical honey group,and combined group showed smaller interdental distances,lower pain intensity,less total dose of analgesics,and lighter facial swelling compared to the control group,with statistically significant differences in the same time of postoperation(P<0.05).Except for the first day after surgery,there was no significant difference in interdental distance between the combined group and the dexamethasone group(P>0.05),and for all other indicators at the same postoperative time,the combined group had better effects than the dexamethasone group or the medical-honey group(P<0.05 to P<0.01).Conclusions:Dexamethasone and medical-honey can reduce the inflammatory complications after mandibular impacted third molar extraction.Both drugs alone or combination can provide a simple,safe,and painless method to eliminate postoperative discomfort,and the combined effect is better.
