摘 要:目的探究血清可溶性抑制肿瘤性蛋白2(soluble suppression of tumorigenicity 2 protein,sST2)、脑钠肽(brain natriuretic peptide,BNP)、白介素-9(interleukin-9,IL-9)在心力衰竭患者中的表达及其与心肾功能的相关性。方法选取心力衰竭患者106例作为心力衰竭组,同时选取同期内在我院体检的健康者80例作为对照组。比较两组sST2、BNP、IL-9、肌酸激酶(creatine kinase,CK)、心肌肌钙蛋白I(cardiac troponin I,CTnI)、左心室射血分数(left ventricular ejection fraction,LVEF)、中性粒细胞明胶酶相关脂质运载蛋白(neutrophil gelatinase-associated lipocalin,NGAL)、血清肌酐(serum creatinine,SCr)、尿素氮(blood urea nitrogen,BUN)水平的差异,并通过ROC曲线分析sST2、BNP、IL-9水平对心力衰竭患者的诊断价值,及sST2、BNP、IL-9水平与心力衰竭患者CK、CTnI、LVEF、NGAL、SCr、BUN的相关性。结果相较于对照组,心力衰竭组CK、CTnI、NGAL、SCr、BUN、sST2、BNP、IL-9更高,LVEF更低(均P<0.05)。ROC曲线分析显示,sST2、BNP、IL-9水平对心力衰竭患者的AUC分别为0.875、0.857、0.757。敏感度为67.0%、89.6%、100%,特异性为92.5%、66.2%、48.7%。Pearson相关性显示,sST2、BNP、IL-9均与LVEF呈负相关(P<0.05),sST2与CK、CTnI、NGAL、SCr、BUN呈正相关(P<0.05),BNP与CK、CTnI、NGAL、SCr、BUN呈正相关(P<0.05),IL-9与CK、CTnI、NGAL、SCr、BUN呈正相关(P<0.05)。结论血清sST2、BNP、IL-9在心力衰竭患者的诊断中具有一定价值,与心力衰竭患者的心肾功能状态密切相关,可作为心力衰竭的生物标志物,帮助医生进行准确的诊断和病情评估。Objective To investigate serum soluble suppression of tumorigenicity 2 protein(sST2),brain natriuretic peptide(BNP),and interleukin-9(IL-9)in the expression in heart failure patients and their correlation with cardiac and renal function.Methods One hundred and six patients with heart failure were selected as the heart failure group,while 80 healthy people who had a physical examination in our hospital during the same period were selected as the control group.The differences were compared in sST2,BNP,IL-9,creatine kinase(CK),cardiac troponin I(CTnI),left ventricular ejection fraction(LVEF),neutrophil gelatinase-associated lipocalin,(NGAL),serum creatinine(SCr)and blood urea nitrogen(BUN).The ROC curve was used to analyze the diagnostic value of sST2,BNP and IL-9 levels in patients with heart failure,and the correlation between sST2,BNP,and IL-9 levels and CK,CTnI,LVEF,NGAL,SCr and BUN in heart failure patients.Results Compared with the control group,the heart failure group had higher CK,CTnI,NGAL,SCr,BUN,sST2,BNP,IL-9,and lower LVEF(P<0.05).The ROC curve analysis showed that the AUC of sST2,BNP,and IL-9 levels in heart failure patients were 0.875,0.857,and 0.757,the sensitivity was 67.0%,89.6%,and 100%,and specificity was 92.5%,66.2%,and 48.7%.Pearson correlation showed that sST2,BNP,and IL-9 were negatively correlated with LVEF(P<0.05),sST2 was positively correlated with CK,CTnI,NGAL,SCr,and BUN(P<0.05),BNP was positively correlated with CK,CTnI,NGAL,SCr,and BUN(P<0.05),and IL-9 was positively correlated with CK,CTnI,NGAL,SCr,and BUN(P<0.05).Conclusion Serum sST2,BNP,and IL-9 are of value in the diagnosis of patients with heart failure,and are closely related to the state of cardiac and renal function in patients with heart failure,and can be used as biomarkers of heart failure to help doctors make accurate diagnosis and assessment of the condition.
