高频超声及临床指标评估五藤冲剂辅助治疗活动期类风湿关节炎疗效的价值

摘　　要：目的探讨高频超声显像及临床指标在评估五藤冲剂辅助甲氨蝶呤治疗活动期类风湿关节炎疗效中的价值。方法将120例活动期类风湿关节炎(RA)患者根据不同治疗方案进行分组,对照组(n=57)口服甲氨蝶呤治疗,观察组(n=63)使用甲氨蝶呤+五藤冲剂联合治疗,对比两组的临床疗效、症状指标、实验室指标、超声指标。结果观察组治疗有效率为95.24%,明显高于对照组的78.95%(P<0.05);观察组治疗后关节压痛数、关节肿胀数、晨僵时间、DSA28评分均低于对照组,握力大于对照组(P<0.05);观察组治疗后RF、CRP、ESR、IL-1β、IL-6、TNF-ɑ等实验室指标均低于对照组(P<0.05);观察组治疗后关节积液分级、滑膜增生分级、滑膜多普勒血流分级等超声指标均低于对照组(P<0.05)。结论五藤冲剂辅助甲氨蝶呤在治疗活动期RA时疗效显著,可有效缓解疼痛、抑制炎症反应,并改善关节功能。通过高频超声和临床指标评估,我们能有效地评定治疗效果、监测关节病变,以指导临床决策。Objective To explore the value of high-frequency ultrasonography and clinical indicators in assessing the efficacy of Wuteng decoction treatment for active rheumatoid arthritis(RA).Methods One hundred and twenty patients with active RA were grouped according to different treatment plans,the control group(n=57)was treated with oral methotrexate,the observation group(n=63)was treated with methotrexate+Wuteng decoction,and the clinical efficacy,symptom index,laboratory index and ultrasound index of the two groups were compared.Results The response rate in the observation group was 95.24%,significantly higher than 78.95%in the control group(P<0.05);the number of joint tenderness,joint swelling,morning stiffness time and DSA 28 score in the observation group were lower than those in the control group,and the grip strength was greater than that in the control group(P<0.05);RF,CRP,ESR,IL-1β,IL-6,and TNF-ɑwere lower in the observation group than those in the control group(P<0.05);the ultrasound indexes,including articular effusion grade,synovial hyperplasia grade and synovial Doppler flow grade in the observation group were lower than those in the control group(P<0.05).Conclusion The Wuteng decoction has shown significant efficacy in the treatment of active RA,effectively relieving pain,suppressing inflammatory responses,and improving joint function.By evaluating high-frequency ultrasonography and clinical indicators,we can effectively assess treatment effects and monitor joint lesions to guide clinical decision-making.
