血清miR-10a、miR-20a、miR-615-3p、CEA、CA19-9联合检测对结直肠癌的诊断价值

摘　　要：目的分析血清微小RNA(miR)-10a、miR-20a、miR-615-3p、癌胚抗原(CEA)、糖基抗原19-9(CA19-9)联合检测对结直肠癌(CRC)的诊断价值。方法回顾性选取2017年10月至2019年2月该院收治的50例CRC患者为CRC组,另选取同期该院收治的肠道良性病变患者50例为疾病对照组,选取同期在该院体检的30例健康体检者为健康对照组。采用实时荧光定量聚合酶链反应检测各组血清miR-10a、miR-20a、miR-615-3p水平,采用电化学发光法检测各组血清CEA、CA19-9水平。采用多因素Logistic回归分析CRC发生的影响因素;绘制受试者工作特征(ROC)曲线评估血清miR-10a、miR-20a、miR-615-3p、CEA、CA19-9对CRC的诊断价值。结果疾病对照组患者血清miR-10a水平低于健康对照组,而血清miR-615-3p、CEA、CA19-9水平高于健康对照组,差异均有统计学意义(P<0.05);CRC组患者血清miR-10a水平低于疾病对照组和健康对照组,而血清miR-20a、miR-615-3p、CEA、CA19-9水平高于疾病对照组和健康对照组,差异均有统计学意义(P<0.05)。不同肿瘤最大径、Dukes分期、细胞分化程度及是否合并淋巴结转移CRC患者的血清miR-10a、miR-20a、miR-615-3p、CEA、CA19-9水平比较,差异均有统计学意义(P<0.05),但不同肿瘤部位及是否规律饮食CRC患者的血清miR-10a、miR-20a、miR-615-3p、CEA、CA19-9水平比较,差异均无统计学意义(P>0.05)。多因素Logistic回归分析结果显示血清miR-10a水平升高是CRC发生的保护因素(P<0.05),血清miR-20a、miR-615-3p、CEA、CA19-9水平升高是CRC发生的危险因素(P<0.05)。ROC曲线结果显示,血清miR-10a、miR-20a、miR-615-3p、CEA、CA19-9联合检测诊断CRC发生的曲线下面积(AUC)为0.952,显著高于各指标单独检测的AUC(Z_(联合-miR-10a)=3.735,P<0.001;Z_(联合-miR-20a)=3.650,P<0.001;Z_(联合-miR-615-3p)=4.133,P<0.001;Z_(联合-CEA)=4.536,P<0.001;Z_(联合-CA19-9)=3.708,P<0.001;Z_(联合-CEA+CA19-9)=3.515,P<0.001),5项血清指标联合检测的灵Objective To analyze the diagnostic value of combined detection of serum microRNA(miR)-10a,miR-20a,miR-615-3p,carcinoembryonic antigen(CEA)and carbohydrate antigen 19-9(CA19-9)in colorectal cancer(CRC).Methods Fifty CRC patients admitted to the hospital from October 2017 to February 2019 were retrospectively collected as the CRC group,while 50 patients with intestinal benign lesions admitted to the hospital during the same period were selected as the disease control group and 30 healthy individuals underwent physical examination in the hospital were regarded as the healthy control group.Real-time fluorescence quantitative polymerase chain reaction was applied to detect serum levels of miR-10a,miR-20a and miR-615-3p;electrochemiluminescence was applied to detect the levels of serum CEA and CA19-9.The influencing factors of CRC were analyzed by Multivariate Logistic regression.Receiver operating characteristic(ROC)curve was applied to analyze the diagnostic value of serum miR-10a,miR-20a,miR-615-3p,CEA and CA19-9 for CRC.Results The serum miR-10a level in the disease control group was obviously lower than that in the healthy control group,while the serum miR-615-3p,CEA and CA19-9 levels were obviously higher than those in the healthy control group,with statistically significant differences(P<0.05).The serum miR-10a level in the CRC group was lower than that in the disease control group and the healthy group,while the levels of serum miR-20a,miR-615-3p,CEA and CA19-9 were higher than those in the disease control group and the healthy group,with statistically significant differences(P<0.05).There were statistically significant differences in serum miR-10a,miR-20a,miR-615-3p,CEA and CA19-9 levels among patients with different tumor maximum diameter,Dukes stage,cell differentiation degree and whether CRC was combined with lymph node metastasis(P<0.05),but there were no statistically significant differences in serum levels of miR-10a,miR-20a,miR-615-3p,CEA and CA19-9 among CRC patients with different tumor sites and whe
