喉罩通气全身麻醉在输尿管镜钬激光碎石术中的应用效果及对应激反应的影响

摘　　要：目的分析喉罩通气全身麻醉在输尿管镜钬激光碎石术中的应用价值。方法按麻醉方法的不同,将2022年1月—2023年12月苏州市中西医结合医院接诊的115例行输尿管镜钬激光碎石术患者分为对照组(57例,行气管插管全身麻醉)、研究组(58例,行喉罩通气全身麻醉)。比较两组应激反应、血流动力学水平、恢复指标及不良反应发生率。结果麻醉即刻,相比于对照组,研究组皮质醇、去甲肾上腺素及肾上腺素水平更低,差异有统计学意义(P均<0.05)。麻醉即刻、术毕即刻,相比于对照组,研究组心率及平均动脉压水平更低,差异有统计学意义(P均<0.05)。相比于对照组,研究组麻醉药物用量更少,术后呼吸恢复时间更短,差异有统计学意义(P均<0.05)。在不良反应发生率方面,研究组[1.72%(1/58)]低于对照组[10.53%(6/57)],差异有统计学意义(χ^(2)=3.896,P<0.05)。结论喉罩通气全身麻醉应用于钬激光碎石术,能够降低患者应激反应,维持血流动力学稳定,促进患者恢复,且具有较高的安全性。Objective To analyze the application value of laryngeal mask ventilation general anesthesia in ureteroscopic holmium laser lithotripsy.Methods According to different anesthesia methods,a total of 115 patients who underwent ureteroscopic holmium laser lithotripsy in Suzhou Integrated Traditional Chinese and Western Medicine Hospital from January 2022 to December 2023 were divided into control group(57 cases,underwent tracheal intubation general anesthesia)and study group(58 cases,underwent laryngeal mask ventilation general anesthesia).The stress response,hemodynamic levels,recovery indexes and adverse reactions between the two groups were compared.Results Immediately after anesthesia,compared with the control group,the levels of cortisol,norepinephrine and epinephrine in the study group were lower,and the differences were statistically significant(all P<0.05).Immediately after anesthesia and immediately after operation,compared with the control group,the heart rate and mean arterial pressure in the study group were lower,and the differences were statistically significant(all P<0.05).Ccompared with the control group,the dosage of anesthetic drugs in the study group was less and the postoperative respiratory recovery time was shorter,the differences were statistically significant(both P<0.05).The incidence of adverse reactions in the study group[1.72%(1/58)]was lower than that in the control group[10.53%(6/57)],and the difference was statistically significant(χ^(2)=3.896,P<0.05).Conclusion Laryngeal mask ventilation general anesthesia applied to holmium laser lithotripsy can reduce the stress response of patients,maintain hemodynamic stability,promote the recovery of patients,and has high safety.
