多层螺旋CT与磁共振成像联合检查对腕关节损伤的诊断价值

摘　　要：目的探讨多层螺旋CT(multi-slice spiral CT,MSCT)与磁共振成像(magnetic resonance imaging,MRI)联合检查对腕关节损伤的诊断价值。方法选取2022年1月—2023年9月栖霞市人民医院收治的100例疑似腕关节损伤患者为研究对象。所有患者均行MSCT、MRI检查,以腕关节镜检查结果为诊断金标准,分析MSCT、MRI及联合检查的诊断效能。结果腕关节镜检出阳性84例,阴性16例;MSCT的阳性检出率为78.00%(78/100);MRI的阳性检出率为82.00%(82/100);联合检查的阳性检出率为84.00%(84/100)。联合检查的灵敏度和准确度高于MSCT、MRI诊断,差异有统计学意义(P均<0.05)。MSCT、MRI及联合检查的特异度、阳性预测值、阴性预测值比较,差异无统计学意义(P均>0.05)。MSCT、MRI及联合检查与腕关节镜检查的一致性分析,kappa=0.419、0.646、0.777。结论联合诊断的灵敏度和准确度高于单一检测,且一致性较高,可有效提升腕关节损伤检出率。Objective To investigate the diagnostic value of multi-slice spiral CT(MSCT)combined with magnetic resonance imaging(MRI)in wrist joint injury.Methods A total of 100 patients with suspected wrist joint injury admitted to Qixia People's Hospital from January 2022 to September 2023 were selected as the research objects.All patients were examined by MSCT and MRI.The diagnostic efficacy of MSCT,MRI and combined examination was analyzed with the results of wrist arthroscopy as the gold standard.Results There were 84 positive cases and 16 negative cases diagnosed by wrist arthroscopy.The positive detection rate of MSCT was 78.00%(78/100),the positive detection rate of MRI was 82.00%(82/100),the positive detection rate of combined examination was 84.00%(84/100).The sensitivity and accuracy of combined examination were higher than those of MSCT and MRI,and the differences were statistically significant(both P<0.05).There were no significant differences in the specificity,positive predictive value and negative predictive value of MSCT,MRI and combined examination(all P>0.05).Consistency analysis of MSCT,MRI and combined examination and wrist arthroscopy,kappa=0.419,0.646,0.777.Conclusion The sensitivity and accuracy of combined examination are higher than those of single detection,and the consistency is high,which can effectively improve the detection rate of wrist joint injury.
