摘 要:目的探讨新生儿肺部疾病患者采用肺部超声技术完成鉴别诊断后的应用价值。方法选取2022年1月—2023年5月山东健康集团枣庄医院收治的43例疑似肺部疾病患儿为研究对象。所有疑似肺部疾病患儿在入院24 h内均接受肺部超声检查,以临床诊断最终结果作为金标准。分析肺部超声技术的诊断效能及诊断符合率。结果临床诊断最终结果诊断阳性38例,阴性5例;肺部超声技术诊断阳性37例,阴性6例。肺部超声技术诊断的灵敏度为94.74%(36/38),特异度为80.00%(4/5),准确度为93.02%(40/43),kappa值为0.688。肺部超声技术对呼吸窘迫综合征疾病、肺炎、新生儿湿肺疾病、新生儿胎粪吸入综合征的诊断符合率分别为90.00%(9/10)、95.00%(19/20)、100.00%(3/3)、100.00%(5/5)。结论肺部超声技术诊断新生儿肺部疾病的诊断效能及诊断符合率较高。Objective To explore the application value of lung ultrasound technology in the differential diagnosis of neonatal pulmonary diseases.Methods A total of 43 children with suspected pulmonary diseases admitted to Shandong Health Group Zaozhuang Hospital from January 2022 to May 2023 were selected as the research objects.All children with suspected pulmonary diseases underwent lung ultrasound examination within 24 hours of admission,and the final results of clinical diagnosis were used as the gold standard.The diagnostic efficiency and diagnostic coincidence rate of lung ultrasound technology were analyzed.Results The final results of clinical diagnosis were positive in 38 cases and negative in 5 cases.Pulmonary ultrasound diagnosis was positive in 37 cases and negative in 6 cases.The sensitivity,specificity and accuracy of lung ultrasound were 94.74%(36/38),80.00%(4/5),93.02%(40/43)and the kappa value was 0.688.The diagnostic coincidence rates of pulmonary ultrasound in respiratory distress syndrome,pneumonia,neonatal wet lung disease and neonatal meconium aspiration syndrome were 90.00%(9/10),95.00%(19/20),100.00%(3/3)and 100.00%(5/5),respectively.Conclusion The diagnostic efficiency and diagnostic coincidence rate of lung ultrasound in the diagnosis of neonatal pulmonary diseases are high.
