螺旋CT技术诊断肠梗阻的应用效果及检出率评价

摘　　要：目的评价在肠梗阻患者的诊断中螺旋CT的应用效果及检出率。方法选取2023年5月—2024年4月黔东南苗族侗族自治州人民医院收治的200例疑似肠梗阻患者为研究对象,均接受螺旋CT和X线检查,以临床综合诊断为金标准,分析两种方式的检出情况以及诊断效能。结果200例疑似肠梗阻患者经临床综合诊断检出阳性191例,阴性9例。X线检出阳性176例,阴性24例。螺旋CT检出阳性186例,阴性14例。螺旋CT的灵敏度以及准确度分别为96.86%(185/191)、96.50%(193/200),均高于X线的91.10%(174/191)、90.50%(181/200),差异有统计学意义(χ^(2)=5.598、5.924,P均<0.05);两种检查方法特异度比较,差异无统计学意义(P>0.05)。螺旋CT在不同肠梗阻病变区域的诊断符合率高于X线,差异有统计学意义(P<0.05)。螺旋CT在肠梗阻发生原因的诊断符合率高于X线,差异有统计学意义(P<0.05)。结论螺旋CT诊断有助于明确肠梗阻的实际情况,可准确鉴别肠梗阻的发生位置以及原因,为后续治疗措施的实施提供可靠依据。Objective To evaluate the application effect and detection rate of spiral CT in the diagnosis of intestinal obstruction.Methods A total of 200 patients with suspected intestinal obstruction admitted to the Qiandongnan Miao and Dong Autonomous Prefecture People's Hospital from May 2023 to April 2024 were selected as the study objects.All of them received spiral CT and X-ray examination,and comprehensive clinical diagnosis was taken as the gold standard.The detection situation and diagnostic efficiency of the two methods were analyzed.Results Among 200 patients with suspected intestinal obstruction,191 cases were positive and 9 cases were negative.X-ray detected 176 positive cases and 24 negative cases.Spiral CT detected 186 positive cases and 14 negative cases.The sensitivity and accuracy of spiral CT were 96.86%(185/191)and 96.50%(193/200),respectively,which were higher than 91.10%(174/191)and 90.50%(181/200)of X-ray,and the differences were statistically significant(χ^(2)=5.598,5.924,both P<0.05).There was no significant difference in the specificity between the two methods(P>0.05).The diagnostic coincidence rate of spiral CT in different ileus lesion areas was higher than that of X-ray,and the difference was statistically significant(P<0.05).The diagnostic coincidence rate of spiral CT in the cause of intestinal obstruction was higher than that of X-ray,and the difference was statistically significant(P<0.05).Conclusion Spiral CT diagnosis is helpful to clarify the actual situation of intestinal obstruction,which can accurately identify the location and causes of intestinal obstruction,and provide a reliable basis for the implementation of follow-up treatment measures.
