二元视角下中重度宫腔粘连患者夫妇生育压力的影响因素混合性研究

摘　　要：目的 探究中重度宫腔粘连患者夫妇的生育压力现况及其影响因素,为临床开展针对性改善中重度宫腔粘连患者夫妇心理状况的干预措施提供新视角和理论依据。方法 采用便利抽样法,选取2023年8至12月在某三级甲等妇产医院就诊的中重度宫腔粘连患者夫妇202对进行问卷调查。随后结合问卷调查结果对明显伴有生育压力的11对夫妇进行半结构式访谈。结果 中重度宫腔粘连夫妇的生育压力处于较高的水平,单因素分析显示,年龄是中重度宫腔粘连患者生育压力的影响因素,居住地和疾病了解程度是影响中重度宫腔粘连夫妇生育压力的共同因素。多元线性回归分析显示,患者的生育压力受其疾病感知情况、抗逆力水平、社会支持及配偶二元应对的影响,配偶的生育压力受患者疾病感知情况、自身抗逆力水平及患者二元应对水平的影响。访谈结果主要提炼出影响夫妇双方生育压力的客观原因、主观因素及夫妇之间相互作用的3个主题。结论 中重度宫腔粘连夫妇生育压力较高且受多种因素相互影响,临床医务人员应关注中重度宫腔粘连患者夫妇的心理健康状态,鼓励双方共同面对生育压力,积极采取措施,改善临床结局。ObjectiveTo explore the current situation of fertility pressure in couples with moderate to severe intrauterine adhesion and its influencing factors,and to provide a new perspective and theoretical basis for conducting clinical intervention to improve the psychological status of couples with moderate to severe intrauterine adhesion.MethodsA total of 202 couples with moderale to severe intrauterine adhesions who were treated in a tertiaryGrade A maternity hospital from August to December 2023 were selected by convenience sampling method for questionnaire survey.Then,the 11 couples with obvious fertility pressure were interviewed in semi-structured manner combined with the results of the questionnaire survey.ResultsThe fertility pressure of couples with moderate to severe intrauterine adhesions was at a higher level.Univariate analysis showed that age was the influencing factor of fertility pressure of patients with moderate to severe intrauterine adhesions,and residence and disease understanding were the common factors affecting fertility pressure of couples with moderate to severe intrauterine adhesions.Multiple linear regression analysis showed that the fertility pressure of patients was affected by their disease perception,resilience level,social support and spouse dual coping.The fertility pressure of spouses was affected by the disease perception of patients,self-resilience level and the dual coping level of patients.The results of the interview mainly extracted three themes influencing the fertility pressure of couples,including the objective reasons,subjective factors and interaction between the couples.Conclusions The fertility pressure of couples with moderate to severe intrauterine adhesions is high and influenced by various factors.Clinical medical staff should pay attention to the mental health status of couples with moderate to severe intrauterine adhesions,encourage both spouses to face fertility pressure together,and take active measures to improve clinical outcomes.
