超重、肥胖夫妇首次体外受精新鲜移植周期胚胎质量及妊娠结局分析

摘　　要：目的 分析单一及合并超重、肥胖夫妇首次体外受精(IVF)新鲜移植周期的胚胎质量及妊娠结局。方法 采用回顾性队列研究分析2016年1月1日至2021年11月1日西北妇女儿童医院生殖中心收治的11 267对首次行IVF夫妇的临床资料。根据体重指数(BMI)将其分为九组:男、女正常体重组(组1);男性超重组(组2);男性肥胖组(组3);女性超重组(组4);男、女超重组(组5);男性肥胖、女性超重组(组6);女性肥胖组(组7);男性超重、女性肥胖组(组8);男、女肥胖组(组9)。分别比较九组的基线资料和IVF-胚胎移植(ET)结局。结果 与组1比较,组8的可用胚胎率显著降低,组5的正常受精率显著降低,组9的优质胚胎率显著降低。结论 IVF夫妇中男、女超重及肥胖均可能影响正常受精及胚胎质量,但对临床妊娠率、流产率、活产率无显著影响。Objective To analyze the embryo quality and pregnancy outcome during the first in vitro fertilization(IVF)fresh transplantation cycle for single and combined overweight and obese couples.Methods A retrospective cohort study was conducted to analyze the clinical data of 11,267 couples admitted to the Reproductive Center of Northwest Women and Children's Hospital and received IVF for the first time from January12016 to November 12021.According to body mass index(BMI),they were divided into male and female nornal weight group(group 1),male overweight group(group 2),male obese group(group 3),female overweight group(group 4),male and female overweight group(group 5),male obese and female overweight group(group 6),female obese group(group 7),male overweight and female obese group(group 8),male and female obese group(group 9).Baseline data and IVF-embryo transfer(ET)outcomes were compared among the nine groups.Results Compared with group 1,the available embryo rate decreased significantly in group 8,the normal fertilization rate decreased in group 5,and the good embryo rate decreased significantly in group 9.Conclusions Overweight and obese in both male and female IVF couples may affect normal fertilization and embryo quality,but they have no significant effect on clinical pregnancy rate,abortion rate and live birth rate.
