摘　　要：目的 探讨宫腔用交联透明质酸钠凝胶(HA-Gel)对中度宫腔粘连(IUA)术后雌激素受体(ER)、孕激素受体(PR)、CD38、CD138表达的影响。方法选取2022年1月至2024年1月广西壮族自治区生殖医院收治的92例中度IUA患者作为研究对象,按照掷硬币法分为对照组和观察组,每组46例。两组均行宫腔镜粘连分离术,对照组术后采用COOK球囊和雌孕激素人工周期法治疗,观察组术后在对照组基础上联合HA-Gel治疗。比较两组临床疗效、子宫内膜厚度、搏动指数(PI)、阻力指数(RI).子宫内膜ER、PR、CD38、CD138表达,以及月经改善率、美国生育协会(AFS)评分、再粘连率。结果观察组治疗总有效率高于对照组(P<0.05)。治疗1个周期后,观察组子宫内膜厚度大于对照组(P<0.05)。治疗1个周期后,观察组PI和RI低于对照组(P<0.05)。治疗1个周期后,观察组子宫内膜ER、PR表达评分高于对照组,CD38、CD138阳性率低于对照组(P<0.05)。观察组月经改善率高于对照组(P<0.05)。治疗1个周期后,观察组AFS评分低于对照组(P<0.05)。随访1年,观察组再粘连率低于对照组(P<0.05)。结论中度IUA患者术后采用HA-Gel治疗可提高治疗效果,降低再粘连风险。ObjectiveTo investigate the effects of intrauterine cross-linked sodium hyaluronale acid gel(HA-Gel)on the expression of estrogen receptor(ER),progesterone receptor(PR),cluster of differentiation 38(CD38),and CD138 after moderate intrauterine adhesions(IUA)surgery.Methods A total of 92 patients with moderate IUA admitted to Guangxi Zhuang Autonomous Region Reproductive Hospital from January 2022 to January 2024 were selected as the research subjects.They were divided into a control group and an observation group using the coin toss method,with 46 cases in each group.Both groups underwent hysteroscopic adhesion separation surgery.The control group received postoperative treatment with a COOK balloon and estrogen-progestin artificial cycle therapy,while the observation group received additional HA-Gel treatment based on the control group.The clinical efficacy,endometrial thickness,pulsatility index(PI),resistance index(RI),endometrial ER,PR,CD38,and CD138 expression,menstrual improvement rate,American fertility society(AFS)score,and re-adhesion rate were compared between the two groups.Results The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).After one treatment cycle,the endometrial thickness in the observation group was greater than that in the control group(P<0.05).After one treatment cycle,the Pl and RI were lower in the observation group than the control group(P<0.05).After one treatment cycle,the expression scores of endometrial ER and PR were higher in the observation group than the control group,while the positive rates of CD38 and CD138 were lower than the control group(P<0.05).The menstrual improvement rate was higher in the observation group than the control group(P<0.05).After one treatment eycle,the AFS score was lower in the observation group than the control group(P<0.05).After 1-year follow-up,the re-adhesion rate was lower in the observation group than the control group(P<0.05).ConclusionsPostoperative treatment with HA-Gel in patients
