摘 要:目的 探讨微小RNA-203a-3p(miR-203a-3p)、趋化因子(CXC基序)配体1(CXCL1)在子宫腺肌症中的表达及与临床病理特征的相关性。方法 选取2020年11月至2023年11月于新疆四七四医院行手术治疗的150例子宫腺肌症患者作为观察组,另选取同院同期行手术治疗的156例子宫肌瘤患者作为对照组。采用实时荧光定量聚合酶链反应(qRT-PCR)法检测血清miR-203a-3p的相对表达水平,采用酶联免疫吸附试验(ELISA)法检测血清CXCL1的表达水平,分析影响子宫腺肌症发生的因素及血清miR-203a-3p、CXCL1水平对患者发生子宫腺肌症的诊断价值。结果 观察组血清miR-203a-3p水平高于对照组、CXCL1水平低于对照组(P<0.05);与局限型、异位腺体分泌期、病灶体积<50 cm^(3)、无内膜息肉的患者比较,弥漫型、异位腺体增殖期、病灶体积≥50 cm^(3)、有内膜息肉的子宫腺肌症患者中血清miR-203a-3p高表达、CXCL1低表达患者占比升高(P<0.05);miR-203a-3p是发生子宫腺肌症的危险因素(P<0.05),CXCL1是发生子宫腺肌症的保护因素(P<0.05);受试者操作特征(ROC)曲线显示,血清miR-203a-3p、CXCL1单独及联合诊断患者发生子宫腺肌症的AUC分别为0.739、0.686、0.794,二者联合优于各自单独诊断(P<0.05)。结论 子宫腺肌症患者血清miR-203a-3p水平升高、CXCL1水平降低,二者表达水平与患者临床病理特征有关,且对疾病的发生有一定的诊断价值。ObjectiveTo investigate the expression of microRNA-203a-3p(miR-203a-3p)and C-X-C motif chemokine ligand 1(CXCL1)in adenomyosis and their correlation with clinical pathological features.MethodsA total of 150 patients with adenomyosis who underwent surgery treatment in Xinjiang474 Hospital from November2020 to November 2023 were collected as the observation group,and a total of 156 patients with uterine fibroids who underwent surgery treatment in the same hospital during the same period were selected as the control group.Real-time fluorescent quantitative polymerase chain reaction(qRT-PCR)method was applied to detect the relative expression level of serum miR-203a-3p.Enzyme-linked immunosorbent assay(ELISA)was applied to determine the expression level of serum CXCL1.The factors influencing the occurrence of adenomyosis and the diagnostic value of serum miR-203a-3pand CXCL1 levels in patients with adenomyosis were analyzed.Results The observation group showed a higher serum miR-203a-3p level and a lower in CXCL1 levels compared to the control group(P<0.05).Compared with patients with limited type,ectopic glandular secretion phase,lesion volume<50 cm^(3),and no endometrial polyps,adenomyosis patients with diffuse type,ectopic glandular proliferation phase,lesion volume≥50 cm^(3),and endometrial polyps showed higher proportions of patients with high serum miR-203a-3p expression and low CXCL1expression(P<0.05).MiR-203a-3p was a risk factor for developing adenomyosis(P<0.05),while CXCL1 was a protective factor for developing adenomyosis(P<0.05).The receiver operating characteristic(ROC)curve showed that the area under curve(AUC)of patients with adenomyosis diagnosed with serum miR-203a-3p and CXCL1alone and in combination was 0.739,0.686,and 0.794,respectively.The combination of the two was better than the individual diagnosis(P<0.05).Conclusions Patients with adenomyosis have increased serum miR-203a-3p level and decreased CXCL.1 level.The expression levels of both are related to the clinical pathological features of
