摘　　要：目的 探讨活血生精方联合显微镜下精索静脉低位结扎术(MIV)对Ⅱ、Ⅲ度精索静脉曲张合并不育患者精液质量及精子DNA碎片率的影响。方法 选取2022年6月至2023年6月河北省沧州中西医结合医院收治的100例Ⅱ、Ⅲ度精索静脉曲张合并不育患者作为研究对象,随机分为手术组(n=50,给予单纯MIV治疗)和联合组(n=50,给予MIV联合活血生精方治疗)。比较两组治疗效果及治疗前后精液质量、精子DNA碎片率、性激素水平。结果 治疗后,联合组总有效率高于手术组,差异具有统计学意义(P<0.05)。治疗后,联合组精子浓度、前向运动精子百分率高于手术组,精子畸形率、精子DNA碎片率低于手术组,差异具有统计学意义(P<0.05)。治疗后,联合组睾酮水平高于手术组,黄体生成素、卵泡刺激素水平低于手术组,差异具有统计学意义(P<0.05)。结论 Ⅱ、Ⅲ度精索静脉曲张合并不育患者采用MIV联合活血生精方治疗,可进一步提升疗效,改善精液质量、精子DNA碎片率及性激素水平。Objective To investigate the effects of Huoxue Shengjing Forula combined with microscopic inguinal varicocelectomy(MIV)on semen quality and sperm DNA fragmentation index in patients with grade Ⅱ and Ⅲ 1varicocelectomy complicated with infertility.MethodsA total of 100 patients with grade Ⅱand Ⅲ varicocele complicated with infertility admitted to Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine from June 2022 to June 2023 were selected as the study subjects and randomly divided into operation group(n=50,receving simple MIV treatment)and combination group(n=50,receving MIV combined with Huoxue Shengjing Formula treatment).The treatment effect,together with semen quality,sperm DNA fragmentation index and sex hormone levels before and after treatment were compared between the two groups.ResultsAfter treatment,the total effective rate of combination group was higher than that of operation group,and the difference was statistically significant(P<0.05).After treatment,the sperm concentration and the percentage of forward motile sperm in the combination group were higher than those in the operation group,and the sperm malformation rate and sperm DNAfragmentation index were lower than those in the operation group,with statistically significant differences(P<0.05).After treatment,the level of testosterone in combination group was higher than those in operation group,luteinizing hormone and follicle stimulating hormone were lower than those in operation group,the differences were statistically significant(P<0.05).Conclusions MIV combined with Huoxue Shengjing Formula can improve the therapeutic effect,semen quality,sperm DNA fragmentation index and sex hormone levels in patients with grade Ⅱ and Ⅲ varicocele complicated with infertility.
