少腹逐瘀汤联合地屈孕酮片对子宫内膜息肉宫腔镜术后患者复发情况及GSDMD、caspases-1水平的影响

摘　　要：目的 探讨少腹逐瘀汤联合地屈孕酮片对子宫内膜息肉宫腔镜术后患者复发情况及Gasdermin家族蛋白D(GSDMD)、半胱氨酸蛋白酶-1(caspases-1)水平的影响。方法 选取2021年3月至2023年5月南京同仁医院收治的80例子宫内膜息肉宫腔镜术后患者作为研究对象,根据不同治疗方法分为对照组(n=38)和观察组(n=42)。对照组给予地屈孕酮片治疗,观察组采用少腹逐瘀汤联合地屈孕酮片治疗。比较两组治疗前后中医证候积分、子宫内膜厚度、GSDMD、caspases-1、增殖细胞核抗原(Ki-67)、炎性因子水平及复发率、不良反应发生率。结果 治疗后,两组中医证候积分和子宫内膜厚度下降,且观察组低于对照组(P<0.05)。术后6个月,观察组复发率低于对照组(P<0.05)。治疗后,两组GSDMD、caspases-1、Ki-67表达水平均降低,且观察组低于对照组(P<0.05)。治疗后,两组白介素(IL)-18、IL-6、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)水平均降低,且观察组低于对照组(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论 少腹逐瘀汤联合地屈孕酮片可改善子宫内膜息肉宫腔镜术后患者临床症状,降低子宫内膜厚度和复发率,可能与下调GSDMD、caspases-1、Ki-67表达水平和抑制炎症反应有关。Objective To investigate the effect of Shaofu Zhuyu Decoction combined with dydrogesterone tablets on recurrence,gasdermin family protein D(GSDMD)and caspases-1 in patients with endometrial polyps after hysteroscopy.MethodsA total of 80 patients with endometrial polyps after hysteroscopy in Nanjing Tongren Hospital from March 2021 to May 2023 were selected as the research objects.According to the different treatment methods,they were divided into the control group(n=38)and the observation group(n=42).The control group was treated with dydrogesterone tablets,and the observation group was treated with Shaofu Zhuyu Decoction combined with dydrogesterone tablets.The traditional Chinese medicine(TCM)syndrome score,endometrialthickness,GSDMD,caspases-1,proliferating cell nuelear antigen(Ki-67),inflammatory factors levels,recumrence rate,and incidence of adverse reactions were compared between the two groups before and after treatment.Results After treatment,the TCMsyndrome score and endometrial thickness in the two groups decreased,and those in the observation group were lower than those in the control group(P<0.05).At 6 months after surgery,the recurrence rate of the observation group was lower than that of the control group(P<0.05).After treatment,the expression levels of GSDMD,caspases-1 and Ki-67 in the two groups were all decreased,and those in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of interleukin(IL)-18,IL-6,tumor necrosis factor-a(TNF-a)and C-reactive protein(CRP)in the two groups were all decreased,and those in the observation group were lower than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions Shaofu Zhuyu Decoction combined with dydrogesterone tablets can improve the clinical symptoms of patients with endometrial polyps after hysteroscopy,reduce endometrial thickness and recurrence rale,which may be related to regulating the expression levels of
