摘　　要：目的采用孟德尔随机化(Mendelian randomization,MR)方法探究多不饱和脂肪酸(Polyunsaturated fatty acid,PUFA)和泌尿系结石(Urinary calculus,UC)的因果关系。方法通过13种脂肪酸(Fatty acid,FA)或其相关比率的汇总统计数据(n=1114999)进行了一项双样本MR研究。UC的汇总统计数据来自FinnGen数据库的R9版本,包括4969例病例和213445例对照。采用MR-Egger回归、加权中位数分析、逆方差加权(Inverse variance weighted,IVW)分析、简单模式分析和加权模型分析方法,评估FA与UC之间的因果关系,通过Cochran's Q统计值评估工具变量(Instrumental variable,IV)的异质性。结果IVW分析显示,PUFA与总FA的比例与UC具有负向因果关系,其比值比(Odds ratio,OR)为0.821(95%CI:0.675~0.998,P=0.047)。ω-6 FA与总FA的比例与UC具有负向因果关系,其OR为0.804(95%CI:0.647~0.999,P=0.049)。未观察到显著的IV异质性或横向多效性。结论增加饮食中ω-6 FA的比例可能有助于降低患者发生UC的风险。Objective To explore the relationship between the polyunsaturated fatty acid(PUFA)and urinary calculus(UC)by Mendelian randomization(MR).Methods A two-sample MR study was conducted using summary statistical data(n=1114999)of 13 fatty acids(FA)or their related ratios.The summary statistical data of UC was sourced from the R9 version of the FinnGen database,including 4969 cases and 213445 controls.The relationship between FA and UC was evaluated using MR-Egger regression,weighted median analysis,inverse variance weighted(IVW)analysis,simple mode analysis,and weighted model analysis.The heterogeneity of instrumental variable(IV)was assessed using Cochran's Q statistical values.Results IVW analysis showed that the ratio of PUFA to total FA was associated with decreased risk of UC,with an odds ratio(OR)of 0.821(95%CI:0.675~0.998,P=0.047).The ratio of omega-6 to total FA was associated with decreased risk of UC,with an OR of 0.804(95%CI:0.647~0.999,P=0.049).No significant IV heterogeneity or lateral pleiotropy was observed.Conclusion Increasing the proportion of omega-6 FA in the diet may help decrease the harm of UC.
