摘  要:目的分析肺移植术后曲霉合并新型冠状病毒感染的治疗策略及其临床过程,并探讨临床药师在肺移植术后患者个体化药物治疗和合理用药中的关键作用。方法本研究选取1例肺移植术后并发曲霉和新型冠状病毒感染的患者,临床药师通过分析潜在的药物相互作用,制定个体化给药方案,并进行血药浓度监测以优化治疗策略。结果药师参与临床诊疗,对患者的用药进行分析并制订了针对性的抗感染治疗方案。在实施先诺特韦/利托那韦治疗的同时,维持了他克莫司及艾沙康唑的血药浓度在治疗窗口内,有效控制了曲霉及新型冠状病毒感染。结论临床药师协助肺移植治疗团队制定个体化给药方案,可促进用药的合理性,减少不良反应的发生,从而凸显了临床药师在临床治疗中的专业价值。Objective To analyze the treatment strategies and clinical process for aspergillus infection complicated by COVID-19 in lung transplant recipients,and to investigate the critical role of clinical pharmacists in individualized medication therapy and rational drug use for patients after lung transplantation.Methods A patient who developed aspergillus infection and COVID-19 after lung transplantation was selected.Clinical pharmacists analyzed potential drug interactions,formulated an individualized medication regimen,and conducted therapeutic drug monitoring to optimize the treatment strategy.Results The involvement of pharmacists in clinical care led to the analysis of the patient's medication and the development of a targeted anti-infection treatment plan.While implementing the treatment with simnotrelvir/ritonavir,the pharmacists maintained the blood concentrations of tacrolimus and isavuconazole within the therapeutic window,effectively controlling both aspergillus infection and COVID-19.Conclusion Clinical pharmacists'assistance in the lung transplantation treatment team in formulating individualized medication regimens can promote rational drug use and reduce the incidence of adverse reactions,which highlights the professional value of clinical pharmacists in clinical treatment.
