急性单侧前庭病患者治疗后水平半规管增益值恢复的影响因素分析

摘　　要：目的探讨急性单侧前庭病(AUVP)患者治疗后水平半规管增益值恢复的影响因素。方法回顾性收集2022年1月至2023年12月杭州市中医院收治的65例AUVP患者,将治疗后3个月视频头脉冲试验中水平半规管增益值未恢复正常(增益值<0.80)且有扫视者作为未恢复组(34例),水平半规管增益值恢复正常(增益值≥0.80)且扫视消失者作为恢复组(31例),比较两组患者临床资料和眩晕障碍量表(DHI)评分、伯格平衡量表(BBS)评分、日常生活能力评定量表(ADL)评分、医院焦虑抑郁量表(HADS)评分,采用单因素和多因素logistic回归分析影响治疗后水平半规管增益值恢复的因素。结果未恢复组病程长于恢复组,康复治疗的开始时间晚于恢复组,差异均有统计学意义(均P<0.01)。治疗后未恢复组BBS评分、ADL评分低于恢复组,DHI评分和HADS评分高于恢复组,差异均有统计学意义(均P<0.05)。单因素logistic回归分析结果显示,水平半规管增益值恢复与病程、治疗后DHI评分、治疗后BBS评分、治疗后ADL评分、治疗后HADS评分有关(均P<0.05)。多因素logistic回归分析发现,病程、治疗后BBS评分、治疗后ADL评分、治疗后HADS评分是水平半规管增益值恢复的独立影响因素(均P<0.05)。结论AUVP患者治疗后水平半规管增益值恢复与病程、治疗后DHI评分、治疗后BBS评分、治疗后ADL评分、治疗后HADS评分有关,病程越长、治疗后HADS评分越高的患者水平半规管增益值越难恢复;治疗后BBS评分、ADL评分高的患者水平半规管增益值较易恢复。Objective To explore the factors affecting the recovery of horizontal semicircular canal gain value of the affected side after treatment among patients with acute unilateral vestibular disease(AUVP).Methods A retrospective analysis was conducted on 65 patients with AUVP admitted to Hangzhou Hospital of Traditional Chinese Medicine from January 2022 to December 2023.Those whose horizontal semicircular canal gain values did not return to normal(gain values<0.80)in video head impulse test 3 months after treatment and had saccades were assigned to the non-recovery group(34 cases),while those whose horizontal semicircular canal gain values returned to normal(gain values≥0.80)with saccades disappearance were assigned to the recovery group(31 cases).The clinical data,and scores of Dizziness Handicap Inventory(DHI),Berg Balance Scale(BBS),Ability of Daily Living Score(ADL),Hospital Anxiety and Depression Scale(HADS)of the two groups were compared,and univariate and multivariate logistic regression analyses were used to determine the factors affecting the recovery of horizontal semicircular canal gain values after treatment.Results Compared with the recovery group,the disease duration of the non-recovery group was longer,and the start time of rehabilitation treatment was later,with statistically significant differences(all P<0.01).The BBS and ADL scores of the non-recovery group were lower than those of the recovery group,while the DHI and HADS scores were higher,with statistically significant differences(all P<0.05).The results of univariate logistic regression analysis showed that the recovery of horizontal semicircular canal gain value was related to disease duration,and post-treatment scores of DHI,BBS,ADL,and HADS(all P<0.05).Further multivariate logistic regression analysis revealed that disease duration,and post-treatment scores of BBS,ADL and HADS were independent influencing factors for the recovery of horizontal semicircular canal gain value(all P<0.05).Conclusion The recovery of horizontal semicircular canal
