妊娠期亚临床甲状腺功能减退的临床分析及甲状腺激素替代治疗的效果观察

摘　　要：目的目的分析临床妊娠期亚临床甲状腺功能减退(SCH)的具体机制,并探究其行甲状腺激素(TH)替代治疗(THRT)的效果。方法方法此次筛选从2023年1月起就诊的妊娠期SCH孕妇,并于2024年6月终止,共选取60例入选,按不同疗法分为THRT组(THRT疗法,30例)和CG组(常规疗法,30例)。比较临床指标变化及分娩不良结局。结果结果两组SCH孕妇治疗后促TH(TSH)水平较治疗前均降低,且THRT组SCH孕妇的TSH水平更低(P<0.05)。两组SCH孕妇治疗后铁蛋白(SF)指标、维生素D2(VD2)指标等较治疗前均改善,且THRT组SCH孕妇的两项指标更优(P<0.05)。两组SCH孕妇的分娩不良结局占比接近(P>0.05)。结论临床妊娠期SCH选用THRT治疗后对分娩不良结局影响较小,且可调节SF指标和VD2指标水平,还可对甲状腺功能予以改善,有借鉴推广优势。Objective To analyze the specific mechanism of subclinical hypothyroidism(SCH)in clinical pregnancy and investigate the effect of thyroid hormone(TH)replacement therapy(THRT).Method In this screening of pregnant women with SCH in pregnancy who were seen from January 2023 and terminated in June 2024,a total of 60 cases were selected for enrollment,and were divided into THRT group(THRT therapy,30 cases)and CG group(conventional therapy,30 cases)according to different therapies.Changes in clinical indicators and adverse birth outcomes were compared.Result The levels of pro-TH(TSH)were lower in SCH pregnant women in both groups after treatment compared with those before treatment,and the levels of TSH were lower in SCH pregnant women in the THRT group(P<0.05).Ferritin(SF)index and vitamin D2(VD2)index of SCH pregnant women in both groups improved after treatment compared with before treatment,and the two indexes of SCH pregnant women in the THRT group were better(P<0.05).The proportion of adverse outcomes of labor and delivery in SCH pregnant women in the two groups was close(P>0.05).Conclusion THRT treatment for SCH in clinical pregnancy has less impact on the adverse outcomes of labor and delivery,and it can regulate the levels of SF indexes and VD2 indexes,as well as improve the function of the thyroid gland,which has the advantage of popularization.
