基于精准护理理念的出院计划能力量表的编制及信效度检验

摘　　要：目的编制适用于评估我国临床护士出院计划能力的量表,并检验其信效度。方法以精准护理理念为指导,通过文献回顾、质性访谈、2轮专家函询进行量表编制,采用便利抽样法,共选取556名临床护士开展问卷调查,采用项目分析法进行条目筛选并形成正式量表。采用探索性因子分析和验证性因子分析进行量表的效度检验,采用Cronbach'sα系数、折半系数和重测信度评价量表的信度。结果探索性因子分析提取3个因子共25个条目,累计方差贡献率为72.631%;验证性因子分析结果显示,χ^(2)/df=5.554,拟合优度指数(GFI)=0.747,比较拟合指数(CFI)=0.918,增量拟合指数(IFI)=0.919,塔克-刘易斯指数(TLI)=0.903,近似误差均方根(RMSEA)=0.117。总量表的Cronbach'sα系数为0.967,折半信度为0.861,重测信度为0.868。量表条目水平的内容效度指数为0.88~1.00,量表水平的平均内容效度指数为0.98。结论护士视角下基于精准护理理念的出院计划能力量表具有良好的信效度,可作为临床护士出院计划能力的评估工具。Objective To compile a scale suitable for assessing the discharge planning competency of clinical nurses in China and to detect its reliability and validity.Methods The scale was compiled through literature review,qualitative interviews,and two rounds of expert consultations guided by the concept of precision nursing.A convenience sampling method was used to select 556 clinical nurses for a questionnaire survey.The item analysis was conducted to screen items and form the formal scale.Exploratory factor analysis(EFA)and confirmatory factor analysis(CFA)were used to detect the validity of the scale.Cronbach'sαcoefficient,split-half coefficient,and test-retest reliability were used to evaluate the reliability of the scale.Results Exploratory factor analysis extracted 3 factors with 25 items,and the cumulative variance contribution rate was 72.631%.Confirmatory factor analysis showed thatχ2/df equaled 5.554,goodness-of-fit index(GFI)equaled 0.747,comparative fit index(CFI)equaled 0.918,incremental fit index(IFI)equaled 0.919,Tucker-Lewis index(TLI)equaled 0.903,and root mean square error of approximation(RMSEA)equaled 0.117.The Cronbach'sαcoefficient of the total scale was 0.967,the split-half reliability was 0.861,and the test-retest reliability was 0.868.The content validity index at the item level ranged from 0.88 to 1.00,and the average content validity index at the scale level was 0.98.Conclusions The discharge planning competency scale based on the concept of precision nursing from the perspective of nurses has good reliability and validity,and it can be used as an assessment tool for the clinical nurses's discharge planning competency.
