预见性护理对连续血液净化相关凝血风险的预防效果

摘　　要：目的分析预见性护理对连续血液净化相关凝血风险的预防效果。方法选取2021年11月—2023年11月南京医科大学附属南京医院接受连续性肾脏替代治疗(CRRT)治疗的患者96例,采用随机数字表法将其分为观察组和对照组,各48例。对照组给予常规护理,观察组给予预见性护理。比较两组凝血风险发生率、凝血功能、护理质量及并发症发生率。结果观察组凝血风险总发生率低于对照组(P<0.05)。护理后,两组凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)短于护理前,凝血酶时间(TT)长于护理前,D-二聚体(D-D)低于护理前(P<0.05);观察组PT、APTT、TT短于对照组,D-D低于对照组(P<0.05)。两组基础护理得分比较,差异无统计学意义(P>0.05);观察组仪器操作准确性、应急处置、风险评估评分高于对照组(P<0.05)。观察组并发症总发生率低于对照组(P<0.05)。结论预见性护理对CRRT相关凝血风险的预防作用较为确切,可有效降低凝血风险和并发症发生率,值得临床应用。Objective To analyze the preventive effect of predictive nursing on blood coagulation risk associated with continuous renal replacement therapy(CRRT).Methods A total of 96 patients receiving CRRT treatment in Nanjing First Hospital,Nanjing Medical University from November 2021 to November 2023 were selected.According to random number table method,the patients were divided into observation group and control group,with 48 cases in each group.The control group was given routine nursing,and observation group was given predictive nursing.The incidence of blood coagulation risk,coagulation function,nursing quality,and complication rate were compared between two groups.Results The total incidence of blood coagulation risk in observation group was lower than that in control group(P<0.05).After nursing,the prothrombin time(PT)and activated partial thromboplastin time(APTT)of two groups were shorter than before nursing,thrombin time(TT)was longer than before nursing,and D-dimer(D-D)was lower than before nursing(P<0.05);PT,APTT,and TT in observation group were shorter than those in control group,while D-D was lower than that in control group(P<0.05).There was no significant difference in basic nursing score between two groups(P>0.05);The scores of accuracy of instrument operation,emergency response,and risk assessment of observation group were higher than those of control group(P<0.05).The total complication rate of observation group was lower than that of control group(P<0.05).Conclusions The preventive effect of predictive nursing on blood coagulation risk related to CRRT is relatively accurate,and it can effectively reduce blood coagulation risk and the incidence of complications,worth clinical application.
