摘　　要：目的总结老年患者下肢动脉硬化闭塞症(ASO)介入治疗的围手术期护理经验。方法回顾性分析2021年6月—2023年9月安徽省芜湖市第二人民医院肿瘤介入科收治的27例老年ASO患者的临床资料,均行下肢动脉血管介入术,总结术前护理、术中护理、术后护理、术后专科宣教、术后并发症护理等方面的护理经验。结果27例行下肢动脉血管介入术治疗患者均顺利完成手术,患者术后疼痛缓解情况较术前明显好转,经过球囊扩张血管后,下肢皮温也有所提升;1例术后出现皮下血肿,经过及时干预得到有效控制;2例发生灌注综合征,经过处理很快好转;4例因下肢坏疽严重,疼痛减轻不明显,转往骨科和烧伤整形科进一步治疗。所有患者均未发生肾功能损害、肺栓塞及支架移位等护理并发症,均好转出院。结论有效的术前、术中和术后护理可以有效地提高老年ASO介入治疗的手术成功率,减少围手术期并发症的发生,提高患者的安全性和治疗效果。Objective To analyze the perioperative nursing experience of elderly patients with lower extremity arteriosclerosis obliterans(ASO)in interventional therapy.Methods The clinical data of 27 elderly ASO patients admitted to Department of Interventional Oncology,the Second People's Hospital,Wuhu,Anhui Province from June 2021 to September 2023 were retrospectively analyzed.All patients underwent lower extremity arterial interventional surgery,and the nursing experience in preoperative nursing,intraoperative nursing,postoperative nursing,postoperative specialist education and postoperative complication nursing was summarized.Results A total of 27 patients underwent lower extremity arterial vascular intervention successfully completed the operation,the postoperative pain relief of patients was significantly better than before,after the balloon dilation of blood vessels,lower extremity skin temperature also increased.Subcutaneous hematoma occurred in 1 patient after operation,which was effectively controlled by timely intervention;Perfusion syndrome occurred in 2 cases and improved quickly after treatment.Due to severe gangrene of lower limbs,4 cases were transferred to orthopedics and burn orthopedics for further treatment.No nursing complications such as renal function impairment,pulmonary embolism and stent displacement occurred in all patients,and all patients were discharged with improvement.Conclusions Effective preoperative,intraoperative and postoperative nursing can effectively improve the surgical success rate of ASO interventional therapy in the elderly,reduce the occurrence of perioperative complications,and improve the safety and therapeutic effect of patients.
