摘 要:目的建立C57BL/6小鼠脓毒症急性肺损伤模型,观察芪归银方对于小鼠急性肺损伤的肺保护作用,并探寻其机制。方法C57BL/6小鼠100只,随机分为假手术组(假手术组)、脓毒症组(模型组)及芪归银方低、中、高剂量组(中药低剂量、中药中剂量、中药高剂量组)各20只;组内分成24、48 h组各10只。假手术组仅开腹操作,其余组均通过盲肠结扎穿孔建立脓毒症肺损伤模型。芪归银方低、中、高剂量组在模型组手术后6 h以中药液灌胃,假手术组同一时间点以纯水灌胃。比较各组各时间点小鼠肺组织病理改变、肺损伤Smith评分以及肺湿/干重比,免疫荧光检测免疫指标CD68的表达情况,ELISA测定小鼠血清中炎症因子白细胞介素-18(IL-18)、肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)的表达情况。结果盲肠结扎穿孔能够成功建立脓毒症肺损伤模型;与假手术组相比,造模24、48 h后,模型组小鼠肺组织损伤加重,肺组织CD68阳性表达量增加,血清中IL-18、IL-1β、TNF-α的表达量增加;与模型组相比,造模24、48 h后,中药低、中、高剂量干预下,肺损伤减轻,肺组织CD68阳性表达量降低,血清中IL-18、IL-1β、TNF-α的表达量降低。其中,造模24 h时间点中药中剂量组相对于中药低剂量和中药高剂量组,肺损伤改善情况更为明显;48 h时间点,中药高剂量组相对于中药低剂量和中药中剂量组肺损伤改善情况更为明显。结论芪归银方可以减轻肺组织病理损伤,同时降低血清中炎症因子的水平。此外,芪归银方起到的肺保护作用可能与减少巨噬细胞过度聚集有关。Objective:To establish an acute lung injury model of sepsis in C57BL/6 mice,and to observe the lung-protective effect of Qiguiyin Decoction on acute lung injury in mice and its mechanism.Methods:Totally100 C57BL/6 mice were randomly divided into 20 mice each in the sham operation group(sham group),the sepsis group(CLP group),and the low,medium,and high dose groups of Qiguiyin Decoction(QGY-L,QGY-M,and QGYH groups);and the groups were divided into 10 mice in each as the 24 h and 48 h groups.The sham group was operated only with open abdomen,and the rest of the groups were perforated by cecum ligation to establish the septic lung injury model.The low,medium and high dose groups of Qiguiyin Decoction were gavaged with Qiguiyin Decoction 6 h after the operation in the model group,and the sham operation group was gavaged with pure water at the same time point.The histopathological changes in the lungs of mice,the Smith score of lung injury,and the wet/dry weight ratio of the lungs were compared among the groups at each time point,the expression of the immunity index CD68 was detected by immunofluorescence,and the expression of the inflammatory factors IL-18,TNF-α,and IL-1βin the serum of mice was determined by ELISA.Results:Ligation and perforation of the cecum could successfully establish a septic lung injury model;compared with the sham group,after 24 h and 48 h of modeling,mice in the CLP group had increased lung tissue injury,increased positive expression of CD68 in lung tissues,and increased expression of IL-18,IL-1β,and TNF-αin serum;compared with the CLP group,after 24 h and 48 h ofmodeling,QGY-L,QGY-M and QGY-H interventions,lung injury damage was reduced,the positive expression ofCD68 in lung tissue was decreased,and the expression of IL-18,IL-1β,and TNF-αin serum was decreased.Among them,the improvement of lung injury was more obvious in the QGY-M group relative to the QGY-L andQGY-H groups at the 24 h time point of modeling,and the improvement of lung injury was more obvious in theQGY-H group relative
