苏杏痰咳合剂治疗慢性阻塞性肺疾病急性加重期(风寒袭肺证)患者的中药临床药学评价

摘　　要：目的观察苏杏痰咳合剂联合西药治疗风寒袭肺证慢性阻塞性肺病急性加重期(AECOPD)的疗效、安全性以及用药依从性。方法80例患者按随机数字表法分为对照组与观察组各40例。对照组接受西医常规治疗,观察组在对照组基础上加上苏杏痰咳合剂治疗,比较两组中医证候、总体临床疗效、症状、炎症指标变化、不良反应发生情况以及中药临床药师介入后患者用药依从性改善情况。结果治疗后观察组患者总有效率为90.00%,高于对照组的82.50%(P<0.05);治疗后组间对比,观察组中医症状总积分及单项评分(喘息、痰量、鼻塞及流涕)、咯痰、喘息气促好转时间、肺啰音消失时间、CAT问卷积分、血清C反应蛋白(CRP)水平及降钙素原(PCT)水平均优于对照组(P<0.05);经中药临床药师干预后,两组患者用药依从性均有改善(P<0.05);两组在治疗过程中均未出现明显不良反应。结论苏杏痰咳合剂治疗AECOPD(风寒袭肺证)疗效确切,能够抑制炎症反应,改善患者临床症状,且具有较高安全性,可提升患者生活质量。此外,中药临床药师宣教能改善患者用药依从性。Objective:To analyze the efficacy and safety of Suxing Tanke Mixture combined with western medicine in treating acute exacerbation of chronic obstructive pulmonary disease(AECOPD)patients with syndrome of wind-cold attacking lung,as well as the changes in medication adherence after intervention by clinical pharmacists specializing in traditional Chinese medicine(TCM).Methods:A total of 80 patients diagnosed with AECOPD and presenting with syndrome of wind-cold attacking lung were randomly divided into a control group and an observation group,with 40 cases in each group.The control group received conventional western medical treatment,while the observation group was added with Suxing Tanke Mixture on top of the control group.The TCM symptom improvement,overall clinical efficacy,symptom changes,inflammatory marker levels,adverse reactions,and improvements in medication adherence after intervention by TCM clinical pharmacists were compared between the two groups.Results:After treatment,the total effective rate in the observation group was 90.00%,which was significantly higher than 82.50%in the control group(P<0.05).After treatment,the observation group had better outcomes in terms of total TCM symptom scores,individual symptom scores(including wheezing,sputum volume,nasal congestion and rhinorrhea),time to improvement in cough,wheezing,and shortness of breath,disappearance of pulmonary rales,COPD Assessment Test(CAT)questionnaire scores,serum C-reactive protein(CRP),and procalcitonin(PCT)levels compared with the control group(P<0.05).After intervention by TCM clinical pharmacists,medication adherence improved significantly in both groups(P<0.05).No adverse reactions occurred during treatment in either group.Conclusion:Suxing Tanke Mixture shows reliable efficacy in treating AECOPD with syndrome of wind-cold attacking lung,which can effectively inhibit inflammatory responses and improve clinical symptoms,with high safety and the ability to enhance patients'quality of life.Additionally,educational interventions by TCM
