耳穴压豆法防治病毒性肺炎机械通气患者谵妄的疗效观察

摘　　要：目的观察耳穴压豆法防治病毒性肺炎机械通气患者谵妄的临床疗效。方法将120例病毒性肺炎有创机械通气患者随机分为对照组与耳穴压豆组各60例,对照组予以常规护理及康复,耳穴压豆组在常规护理及康复的基础上给予耳穴压豆法。观察两组患者谵妄首次发生时间、持续时间、第4天及第10天谵妄发生率,使用镇痛镇静药天数,身体约束时长的情况。结果治疗组谵妄发生时间晚于对照组,且持续时间及治疗后第4天、第10天的谵妄发生率均低于对照组,差异有统计学意义(P<0.05)。两组患者住院期间使用镇痛镇静药天数低于对照组,差异有统计学意义(P<0.05)。治疗组身体约束时长短于对照组,差异有统计学意义(P<0.05)。结论耳穴压豆法用于防治病毒性肺炎有创机械通气患者谵妄的效果显著,可很好改善临床预后,加快患者康复。Objective:To observe the clinical efficacy of auricular acupressure in preventing and treating delirium in mechanically ventilated patients with viral pneumonia.Methods:A total of 120 mechanical ventilated patients with viral pneumonia were randomly divided into a control group and an auricular acupressure group,60 patients in each group.The control group received routine care and rehabilitation,while the auricular acupressure group received auricular acupressure in addition to routine care and rehabilitation.The onset time,duration,and incidence of delirium on the 4th and 10th days,the number of days using analgesics and sedatives,the duration of physical restrain were observed in both groups.Results:The onset of delirium in the treatment group was later than in the control group,and the duration and incidence of delirium on the 4th and 10th days after treatment were significantly lower than in the control group(P<0.05).The number of days using analgesics and sedatives was significantly lower in the treatment group than in the control group(P<0.05).The duration of physical restraint was significantly shorter in the treatment group than in the control group(P<0.05).Conclusion:Auricular acupressure has a significant effect in preventing and treating delirium in mechanically ventilated patients with viral pneumonia,which can improve clinical outcomes and accelerate patient recovery.
