摘  要:目的:探究风险预警模式下关键流程优化在预防呼吸衰竭患者呼吸机相关肺炎(VAP)中的应用效果。方法:以风险预警模式下关键流程优化干预在医院呼吸与危重症医学科推行时间(2021年9月)为分界点,分为推行前组和推行后组,每组各48例。比较两组患者治疗情况、不良事件发生情况、措施执行率、措施合格率、护理舒适度及护理满意度。结果:推行后组患者通气时间及ICU停留时间均显著少于推行前组,差异均有统计学意义(t=3.975、7.104,P<0.05);推行后组患者VAP发生率、不良事件发生率及投诉率均显著低于推行前组,差异均有统计学意义(χ^(2)=4.909、4.019、4.019,P<0.05);推行后组防控措施执行率、措施合格率显著高于推行前组,差异均有统计学意义(χ^(2)=14.950、9.653、5.658、15.845、6.008、4.042、5.352、4.376,P<0.05);推行后组患者GCQ评分(心理、社会、环境及生理)显著高于推行前组,差异均有统计学意义(t=3.025、2.179、2.306、3.515,P<0.05);推行后组患者满意度高于推行前组,差异有统计学意义(χ^(2)=19.25,P<0.05)。结论:风险预警模式下关键流程优化干预可有效缩短呼吸衰竭患者通气时间,防治VAP,且可提高防治措施执行率与有效率,提高护理满意度。Objective:To explore the application of key process optimization intervention under risk early warning model in the prevention of ventilator-associated pneumonia(VAP)in patients with respiratory failure.Methods:Taking the implementation time of key process optimization intervention under risk early warning model in PCCM in the hospital with September 2021 as the dividing line.Patients were divided into pre-implementation group and post-implementation group.The treatment status,occurrence of adverse events,implementation rates of measures,qualified rates of measures,nursing comfort and nursing satisfaction were compared between both groups of patients.Results:The ventilation time and ICU stay in post-implementation group were significantly shorter than those in pre-implementation group(t=3.975,7.104;P<0.05).The incidence rate of VAP,incidence rate of adverse events and complaint rate in post-implementation group were significantly lower than those in pre-implementation group(χ^(2)=4.909,4.019,4.019;P<0.05).The implementation rate and qualified rate of prevention and control measures in the post-implementation group were significantly higher than those in the pre-implementation group,with statistically significant difference(χ^(2)=14.950,9.653,5.658,15.845,6.008,4.042,5.352,4.376;P<0.05).The GCQ score(psychological,social,environmental and physiological)of the group after implementation was significantly higher than that of the group before implementation,with statistically significant difference(t=3.025,2.179,2.306,3.515;P<0.05).The satisfaction of patients in the post-implementation group was higher than that in the pre-implementation group with statistically significant difference(χ^(2)=19.25,P<0.05).Conclusion:Key process optimization intervention under risk early warning model can effectively shorten the ventilation time of patients with respiratory failure,prevent and treat VAP,and improve the implementation rate and efficiency of prevention and treatment measures,and enhance the nursing satisfaction.
