摘  要:目的:完全清醒局部麻醉无止血带(WALANT)技术下关节镜治疗创伤后髌骨脱位的疗效,探讨其在临床应用中的有效性和安全性。方法:回顾性分析2013年10月—2023年9月北京市回民医院接受WALANT下关节镜治疗的31例创伤性髌骨脱位患者临床资料。采用WALANT结合关节镜下内侧支持带紧缩缝合和外侧支持带松解术进行治疗。术后进行铰链式护膝固定和康复训练,并采用AKS评分和Kujala评分评估疗效。结果:术后6个月随访显示,J-sign征和髌骨恐惧实验均有明显改善,差异有统计学意义(χ^(2)=27.011,P<0.05)。AKS评分和Kujala评分较术前显著提高,差异均有统计学意义(t=40.322、51.272;P<0.05)。表明术后功能较术前明显改善。WALANT技术减少了麻醉并发症,提高了手术室的周转效率。结论:WALANT结合关节镜治疗创伤后髌骨脱位是一种有效的治疗方法,能够显著改善患者的膝关节功能和稳定性,且具有创伤小、恢复快的优点。Objective:To analyze the efficacy of arthroscopic treatment for post-traumatic patellar dislocation under wide awake local anesthesia no tourniquet(WALANT)technology,and to explore its effectiveness and safety in clinical application.Methods:A retrospective analysis was conducted on 31 patients with traumatic patellar dislocation who underwent arthroscopic treatment under WALANT anesthesia in Beijing Huimin Hospital from October 2013 to October 2021.Arthroscopic treatment included medial patellar retinacular tightening and lateral release.Hinged knee brace immobilization and rehabilitation training were implemented,and outcomes were assessed with AKS and Kujala scores.Results:6 months after surgery,the follow-up results showed that there were significant improvements in the J-sign and patellar apprehension test(χ^(2)=27.011,P<0.05),and both AKS and Kujala scores increased significantly compared with preoperative levels(t=40.322,51.272;P<0.05),indicating a marked improvement in postoperative function.The use of WALANT anesthesia reduced anesthesia-related complications and improved the turnover efficiency of the operating room.Conclusion:WALANT combined with arthroscopy for the treatment of post-traumatic patellar dislocation is an effective treatment method,which can significantly improve the function and stability of the patient’s knee joint.It has the advantages of less trauma and faster recovery.
