麦角新碱联合卡前列素氨丁三醇对前置胎盘剖宫产产后出血产妇凝血功能、血清SOD、MDA水平的影响

摘　　要：目的:探讨麦角新碱联合卡前列素氨丁三醇对前置胎盘剖宫产产后出血产妇凝血功能、血清超氧化物歧化酶(SOD)、丙二醛(MDA)水平的影响。方法:选取2021年5月—2022年5月安阳市妇幼保健院收治的68例前置胎盘剖宫产产后出血产妇作为研究对象,随机将其分为对照组和观察组,每组各34例。两组产妇均予以麦角新碱联合卡前列素氨丁三醇治疗,对照组产妇采用常规围产期护理,观察组产妇采用多学科团队(MTD)护理。统计两组产妇产后出血发生情况,比较两组产妇干预前后凝血功能状况、血流动力学特征及血清SOD、MDA、谷胱甘肽过氧化物酶(GSH-px)水平。结果:观察组产妇产后出血发生率为5.88%,低于对照组的26.47%,差异有统计学意义(χ^(2)=7.250,P<0.05)。干预后,观察组产妇FIB、D-D水平较对照组低,差异均有统计学意义(t=9.709、13.117,P<0.05)。干预后,观察组产妇全血黏度低切、全血黏度高切、红细胞压积水平较对照组低,差异均有统计学意义(t=10.115、9.151、7.353,P<0.05)。干预后,观察组产妇SOD、GSH-px水平较对照组高,MDA水平较对照组低,差异均有统计学意义(t=7.376、7.988、6.983,P<0.05)。结论:麦角新碱联合卡前列素氨丁三醇应用于前置胎盘剖宫产产后出血产妇中,可改善其凝血功能,避免其产后过量出血,同时配合MTD护理干预还可改善其用药疗效,促进其血流动力学恢复。Objective:To explore the effect of ergometrine combined with carboprost tromethamine on coagulation function,serum superoxide dismutase(SOD),and malondialdehyde(MDA)levels in postpartum hemorrhage after cesarean section with placenta previa.Methods:68 postpartum hemorrhage patients with placenta previa who underwent cesarean section in Obstetrics and Gynecology Department of the hospital from May 2021 to May 2022 were randomly divided into control group and observation group,with 34 patients in each group.Both groups were treated with ergometrine combined with carboprost tromethamine.The control group received routine nursing based on this,while the observation group received perinatal nursing intervention.The amount of postpartum hemorrhage in both groups was counted,and the coagulation function,hemodynamic characteristics and serum SOD,MDA,GSH-px levels were observed before and after the intervention.Results:The incidence of postpartum hemorrhage in the observation group was 5.88%,which was lower than 26.47%in the control group,and the difference was statistically significant(χ^(2)=7.250,P<0.05).After intervention,FIB and D-D levels in the observation group were lower than those in the control group,and the differences were statistically significant(t=9.709,13.117;P<0.05).After intervention,whole blood viscosity was lower,whole blood viscosity was higher and hematocrit level was lower in observation group than in control group,and the differences were statistically significant(t=10.115,9.151,7.353;P<0.05).After intervention,SOD and GSH-px levels in observation group were higher than those in control group,and MDA levels were lower than those in control group,with statistically significant difference(t=7.376,7.988,6.983;P<0.05).Conclusion:The application of ergometrine combined with carboprost tromethamine in the treatment of postpartum hemorrhage after cesarean section with placenta previa can improve the therapeutic effect,and promote the hemodynamic recovery.
