床旁血液超滤治疗急性左心衰竭合并肾功能不全的临床效果分析

摘　　要：目的:分析床旁血液超滤治疗急性左心衰竭合并肾功能不全的临床效果。方法:选取2020年9月—2022年10月宁津县人民医院收治的急性左心衰竭合并肾功能不全患者60例作为研究对象,随机分为对照组和观察组,每组30例。对照组给予常规治疗,观察组在对照组基础上给予床旁血液超滤治疗。比较两组治疗效果。结果:治疗后,两组尿素氮、肌酐水平降低,且观察组低于对照组(P<0.05);两组左室收缩末期内径、左室舒张末期内径减小,且观察组小于对照组(P<0.05);两组左室射血分数升高,且观察组高于对照组(P<0.05)。治疗后,两组呼吸困难、气促、发绀、咳嗽咳痰评分均降低,且观察组低于对照组(P<0.05)。观察组治疗总有效率高于对照组,并发症总发生率比对照组低(P<0.05)。结论:床旁血液超滤治疗急性左心衰竭合并肾功能不全的临床效果较好,能够改善患者的心、肾功能,缓解其临床症状,降低并发症发生率。Objective:To analyze the clinical effect of bedside blood ultrafiltration treatment for acute left heart failure with renal insufficiency.Methods:From September 2020 to October 2022,60 patients with acute left heart failure and renal insufficiency admitted to Ningjin People's Hospital were selected as the study subjects.They were randomly divided into control group and observation group,with 30 cases in each group.The control group received conventional treatment,and the observation group received bedside blood ultrafiltration treatment on the basis of the control group.The treatment effects were compared between the two groups.Results:After treatment,the levels of urea nitrogen and creatinine in the two groups decreased and the levels in the observation group were lower than those in the control group(P<0.05).The left ventricular end-systolic diameter and the left ventricular end-diastolic diameter in the two groups decreased,and these parameters in the observation group were lower than those in the control group(P<0.05).The left ventricular ejection fraction in the two groups increased,and this parameter in the observation group was higher than that in the control group(P<0.05).After treatment,the scores of dyspnea,shortness of breath,cyanosis,cough and expectoration in the two groups reduced,and the scores in the observation group were lower than those in the control group(P<0.05).The total effective rate of treatment in the observation group was higher than that in the control group,and the total incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion:Bedside blood ultrafiltration has a good clinical effect in the treatment of acute left heart failure with renal insufficiency,and can improve the heart and kidney functions of patients,alleviate their clinical symptoms,and reduce the incidence of complications.
