彩色多普勒超声检查在老年女性脂肪肝健康体检筛查中的应用价值

摘　　要：目的:探究彩色多普勒超声检查在老年女性脂肪肝健康体检筛查中的应用价值。方法:选取2022年6月—2023年12月在吉林延边朝医医院体检科进行健康体检的200名疑似脂肪肝老年女性为研究对象,均进行彩色多普勒超声检查。统计200名老年女性确诊脂肪肝人数以及脂肪肝病变程度情况;以临床综合检查结果为“金标准”,计算彩色多普勒超声检查与临床综合检查结果的一致性,评估彩色多普勒超声筛查诊断老年女性脂肪肝的效能;比较不同脂肪肝病变程度患者肝脏皮下脂肪层厚度、声速值。结果:临床综合检查结果显示,200名老年女性确诊脂肪肝113例,轻度脂肪肝52例,中度脂肪肝36例,重度脂肪肝25例。彩色多普勒超声检查与临床综合检查结果的一致性较好(Kappa=0.914,P<0.001)。彩色多普勒超声检查诊断老年女性脂肪肝的灵敏度、特异度以及准确率分别为98.23%(111/113)、98.85%(86/87)、98.50%(197/200),与临床综合检查结果比较,差异无统计学意义(P>0.05)。不同脂肪肝病变程度患者肝脏皮下脂肪层厚度比较,轻度脂肪肝患者<中度脂肪肝患者<重度脂肪肝患者;不同脂肪肝病变程度患者声速值比较,轻度脂肪肝患者>中度脂肪肝患者>重度脂肪肝患者;差异有统计学意义(P<0.05)。结论:彩色多普勒超声检查无创、操作简便,诊断老年女性脂肪肝的效能较高,在老年女性健康体检中应用彩色多普勒超声筛查,有助于医师诊断脂肪肝,鉴别脂肪肝病变程度。Objective:To explore the application value of color Doppler ultrasound in the diagnosis of fatty liver in elderly women.Methods:A total of 200 elderly women with suspected fatty liver who underwent health checkups at the Physical Examination Department of Jilin Yanbian Chaoyi Hospital from June 2022 to December 2023 were selected as the study subjects.All the subjects underwent color Doppler ultrasound examination.The number of women with a confirmed diagnosis of fatty liver among 200 elderly women and the degree of fatty liver lesions were counted.The comprehensive clinical examination results were used as the"gold standard"to calculate the consistency between the color Doppler ultrasound and pathological examination results,and the diagnostic efficiency of color Doppler ultrasound screening for fatty liver in elderly women was evaluate.The liver subcutaneous fat layer thickness and sound velocity values in patients with different degrees of fatty liver lesions were compared.Results:comprehensive clinical examination results showed that,among 200 elderly women,113 cases were diagnosed with fatty liver,including 52 cases of mild fatty liver,36 cases of moderate fatty liver and 25 cases of severe fatty liver.The color Doppler ultrasound was well consistent with the comprehensive clinical examination results(Kappa=0.914,P<0.001).The sensitivity,specificity and accuracy of color Doppler ultrasound in the diagnosis of fatty liver in elderly women were 98.23%(111/113),98.85%(86/87)and 98.50%(197/200),respectively,and there was no statistically significant difference compared with comprehensive clinical examination results(P>0.05).The t liver subcutaneous fat layer thickness in patients with different degrees of fatty liver lesions was compared and ranked as follows:patients with mild fatty liver<patients with moderate fatty liver<patients with severe fatty liver.The sound velocity values in patients with different degrees of fatty liver lesions was compared and ranked as follows:patients with mild fatty liver>patients
