摘　　要：目的:分析医院-社区-家庭联动管理在老年高血压患者中的应用效果。方法:选取2021年9月—2023年9月黔东南苗族侗族自治州人民医院及示范点收治管理的300例老年高血压患者作为研究对象,随机分为对照组和观察组,各150例。对照组采取常规管理,观察组在对照组基础上采用医院-社区-家庭联动管理。对比两组血压水平、遵医行为及高血压认知水平。结果:管理后,两组收缩压、舒张压均降低,且观察组低于对照组(P<0.05);两组药物治疗、饮食控制、自我血压监测评分均升高,且观察组高于对照组(P<0.05);两组发病原因、疾病预防、血压控制认知评分均升高,且观察组高于对照组(P<0.05)。结论:医院-社区-家庭联动管理模式在老年高血压患者中的应用效果显著,可控制患者血压水平,提升其遵医行为及高血压认知水平。Objective:To analyze the application effect of hospital-community-family linkage management in elderly hypertensive patients.Methods:A total of 300 elderly hypertensive patients who received treatment at the People's Hospital of Qiandongnan Miao and Dong Autonomous Prefecture from September 2021 to September 2023 were selected as the study subjects.They were randomly divided into control group and observation group,with 150 patients in each group.The control group received routine management,while the observation group received hospital-community-family linkage management on the basis of control group.The blood pressure levels,medical compliance behaviors and hypertension cognition levels were compared between two groups.Results:After management,systolic blood pressure and diastolic blood pressure in two groups decreased,and these parameters in the observation group were lower than those in the control group(P<0.05).The scores of drug therapy,diet control and self-blood pressure monitoring in two groups increased,and the scores in the observation group were higher than those in the control group(P<0.05).The scores of onset cause,disease prevention,and blood pressure control cognition in two groups increased,and the scores in the observation group were higher than those in the control group(P<0.05).Conclusion:The hospital-community-family linkage management mode has a significant application effect in elderly patients with hypertension,and can control the blood pressure levels of patients and improve their medical compliance behaviors and hypertension cognition levels.
