同种异体脱细胞真皮在新生儿脐膨出中应用的安全性分析

摘　　要：分析同种异体脱细胞真皮在新生儿脐膨出中应用的安全性。回顾性分析广西壮族自治区妇幼保健院2014年3月―2022年12月35例脐膨出新生儿病例资料,所有患儿均采取同种异体脱细胞真皮修补术治疗。统计患儿治疗成功率、手术前后炎症指标及并发症发生情况。研究结果显示,35例患儿同种异体脱细胞真皮修补术均成功,顺利出院,治愈率为100%,并发症发生率为17.1%。术后第1、3、5d患儿的白细胞水平均显著低于术前(P<0.05);术后第1、3d患儿的C反应蛋白水平均显著高于术前(P<0.05),而术后第5d患儿的C反应蛋白水平高于术前,但差异不显著(P>0.05);术后第1d患儿的中性粒细胞比值高于术前,术后第3、5d患儿的中性粒细胞比值低于术前,但差异均不显著(P>0.05)。研究发现,同种异体脱细胞真皮在新生儿脐膨出的缺损修补中具有较理想的应用效果,排斥反应少,植入安全性较高,值得推广应用。To analyze the safety of allogeneic decellularized dermis in the treatment of neonatal omphalocele.Retrospective analysis of 35 cases of umbilical hernia in newborns at Maternal and Child Health Care of Guangxi Zhuang Autonomous Region from March 2014 to December 2022,all of whom underwent allogeneic decellularized dermal repair surgery.Statistics on the success rate of treatment,inflammation indicators before and after surgery,and incidence of complications in pediatric patients.The research results showed that all 35 pediatric patients underwent allogeneic decellularized dermal repair surgery successfully and were discharged smoothly.The cure rate was 100%,and the incidence of complications was 17.1%.The white blood cell levels of patients on postoperative days 1,3,and 5 were significantly lower than those before surgery(P<0.05);The levels of C-reactive protein in patients on postoperative day 1 and 3 were significantly higher than those before surgery(P<0.05),while the levels of C-reactive protein in patients on postoperative day 5 were higher than those before surgery,but the difference was not significant(P>0.05);The neutrophil ratio of patients on the first day after surgery was higher than before surgery,and the neutrophil ratio of patients on the third and fifth days after surgery was lower than before surgery,but the differences were not significant(P>0.05).Research has found that allogeneic decellularized dermis has ideal application effects in repairing defects of neonatal omphalocele,with few rejection reactions and high implantation safety,and is worthy of promotion and application.
