基于ORTCC模型的营养管理在老年慢性心力衰竭合并失能患者中的应用分析

摘　　要：探讨基于ORTCC模型的精细化营养管理在老年慢性心力衰竭失能患者中的应用效果。选取广西壮族自治区江滨医院2022年1月―2023年8月符合入组条件的60例慢性心力衰竭患者为研究对象,按随机抽样法将其分为对照组和观察组两组,每组各30例,对照组实施常规护理,观察组在常规护理的基础上实施基于ORTCC模型(包含目标、规则、训练、考核和文化5个要素)的精细化营养管理。入院1个月后,对比两组患者的营养状况、生活质量和心肺功能。研究结果显示,干预后,观察组患者的前白蛋白、白蛋白水平及运动耐力指标均显著高于对照组(P<0.05),明尼苏达心力衰竭生活质量评分(MLHFQ)显著低于对照组(P<0.05)。研究发现,应用基于ORTCC模型的精细化营养管理有利于改善老年慢性心力衰竭失能患者的营养状况和生活质量,提高其心肺运动耐力,值得临床推广。To explore the application effect of refined nutritional management based on the ORTCC model in elderly patients with chronic heart failure and disability.Sixty patients with chronic heart failure who met the inclusion criteria at Jiangbin Hospital of Guangxi Zhuang Autonomous Region from January 2022 to August 2023 were selected as the research subjects.They were randomly divided into a control group and an observation group,with 30 patients in each group.The control group received routine nursing care,while the observation group received refined nutritional management based on the ORTCC model(including five elements:goals,rules,training,assessment,and culture)on the basis of routine nursing care.One month after admission,compare the nutritional status,quality of life,and cardiopulmonary function between the two groups of patients.The research results showed that after intervention,the levels of prealbumin,albumin,and exercise endurance indicators in the observation group were significantly higher than those in the control group(P<0.05),and the Minnesota Heart Failure Quality of Life Score(MLHFQ)was significantly lower than that in the control group(P<0.05).Research has found that the application of refined nutrition management based on the ORTCC model is beneficial for improving the nutritional status and quality of life of elderly patients with chronic heart failure and disability,enhancing their cardiorespiratory exercise endurance,and is worthy of clinical promotion.
