摘 要:目的:基于美国国家健康与营养调查(NHANES)数据库研究慢性阻塞性肺疾病(COPD)患者饮食炎症指数(DII)与肌少症的关系。方法:以1999-2006年、2012-2018年NHANES的COPD患者为研究对象,收集一般资料、DII、肌少症情况等数据。根据DII四分位数将COPD患者分为Q1~Q4组。采用Logistic回归分析DII与COPD患者肌少症风险的关系。采用限制性立方样条分析DII与肌少症风险的剂量-效应关系。结果:529例COPD患者中肌少症121例(22.87%)。校正年龄、种族、性别、BMI、饮酒、体力活动后,相比Q1组,Q3组、Q4组的肌少症风险增高,OR(95%CI)分别为2.136(1.121~4.071)、2.779(1.571~4.916);Q1、Q2、Q3、Q4组COPD患者的肌少症风险呈上升趋势(P<0.05);DII≥2.51时DII与肌少症呈剂量-反应关系(P_(非线性)=0.030)。结论:DII≥2.51时,随着DII增加,COPD患者肌少症风险呈增高趋势。Aim:To study the relationship between dietary inflammatory index(DII)and sarcopenia in patients with chronic obstructive pulmonary disease(COPD)based on the National Health and Nutrition Examination Survey(NHANES)database.Methods:Data on general information,DII,and sarcopenia were collected of COPD patients from NHANES during 1999-2006 and 2012-2018.COPD patients were categorized into Q1 to Q4 groups based on DII quartiles.Logistic regression was used to analyze the relationship between DII and the risk of sarcopenia in COPD patients.Restricted cubic splines was used to analyze the dose-effect relationship between DII and the risk of sarcopenia.Results:The prevalence of sarcopenia was 22.87%(121/529).Logistic analysis showed that after adjusting for age,race,gender,BMI,drinking,and physical activity,the risk of sarcopenia was higher in groups Q3 and Q4 compared with group Q1,and OR(95%CI)were 2.136(1.121-4.071),2.779(1.571-4.916).The risk of sarcopenia tended to increase in COPD patients in groups Q1,Q2,Q3,and Q4(P<0.05).DII and sarcopenia showed dose-effect relationship when DII≥2.51(P_(non-linearity)=0.030).Conclusion:The risk of sarcopenia in COPD patients increases with the increase of DII when DII≥2.51.
